Research Protocol LifeSteps for PrEP for young men who have sex with men: adapting a PrEP adherence intervention for high-risk youth

چکیده
Background: New HIV infections occur at a disproportionately high rate among young men who have sex with men (YMSM). It is therefore essential that comprehensive HIV prevention strategies, specifically tailored to their needs and perceptions, are developed, tested, and disseminated. Antiretroviral pre-exposure prophylaxis (PrEP) is effective in decreasing HIV transmission among MSM; however, adherence is critical to its efficacy. In open-label studies among YMSM, adherence was suboptimal. Behavioral approaches that address the unique challenges to YMSM PrEP adherence are needed. Objective: To describe the protocol for intervention refinement and a pilot randomized controlled trial (RCT) of a PrEP adherence intervention, LifeSteps for PrEP for YMSM (LSPY). Methods: This study includes two phases: 1) formative qualitative interviews with approximately 20 YMSM and 10 key informants, for the purposes of intervention adaptation and refinement; and 2) a pilot RCT of up to 50 YMSM to assess feasibility, acceptability, and preliminary efficacy of the LifeSteps for PrEP for YMSM, compared to the PrEP standard of care, to improve PrEP adherence. Participants will be recruited at 3 iTech Subject Recruitment Venues in the United States. Results: Phase 1 is expected to begin in late 2017, and enrollment of Phase 2 is anticipated to begin in early 2018. Conclusions: There are few rigorously developed and tested interventions designed to increase PrEP adherence among YMSM in community settings, despite this population’s high HIV incidence. The long-term goal of this intervention is to develop scalable protocols to optimize atrisk YMSM’s PrEP uptake and adherence, in order to decrease HIV incidence. Trial Registration: The study is in the process of being registered on ClinicalTrials.gov.

Potential impact of pre-exposure prophylaxis for female sex workers and men who have sex with men in Bangalore, India: a mathematical modelling study
INTRODUCTION In Bangalore, new HIV infections of female sex workers and men who have sex with men continue to occur, despite high condom use. Pre-exposure prophylaxis (PrEP) has high anti-HIV efficacy for men who have sex with men. PrEP demonstration projects are underway amongst Indian female sex workers. We estimated the impact and efficiency of prioritizing PrEP to female sex workers and/or ...
متن کاملClinical Effectiveness and Cost-Effectiveness of HIV Pre-Exposure Prophylaxis in Men Who Have Sex with Men: Risk Calculators for Real-World Decision-Making
BACKGROUND Oral pre-exposure prophylaxis (PrEP) can be clinically effective and cost-effective for HIV prevention in high-risk men who have sex with men (MSM). However, individual patients have different risk profiles, real-world populations vary, and no practical tools exist to guide clinical decisions or public health strategies. We introduce a practical model of HIV acquisition, including bo...
متن کاملThe Potential Impact of Pre-Exposure Prophylaxis for HIV Prevention among Men Who Have Sex with Men and Transwomen in Lima, Peru: A Mathematical Modelling Study
BACKGROUND HIV pre-exposure prophylaxis (PrEP), the use of antiretroviral drugs by uninfected individuals to prevent HIV infection, has demonstrated effectiveness in preventing acquisition in a high-risk population of men who have sex with men (MSM). Consequently, there is a need to understand if and how PrEP can be used cost-effectively to prevent HIV infection in such populations. METHODS A...
متن کاملAlcohol-antiretroviral interactive toxicity beliefs as a potential barrier to HIV pre-exposure prophylaxis among men who have sex with men
INTRODUCTION Pre-exposure prophylaxis (PrEP) offers as much as 90% protection against HIV transmission. However, the effectiveness of PrEP depends on uptake and adherence to even intermittent dosing. Along with intoxication leading to unintentional non-adherence, believing that alcohol mixed with pharmaceuticals is harmful (i.e., interactive toxicity beliefs) may lead to poor uptake and intenti...
متن کاملResearch Protocol Developing a mobile app to support linkage to HIV/STI testing and PrEP for young men who have sex with men: LYNX pilot study protocol (ATN 140)
Background: Young men who have sex with men (YMSM) in the US have amongst the highest HIV/sexually transmitted infections (STIs) incidence and the lowest uptake of HIV/STI testing and preexposure prophylaxis (PrEP). Nearly universal mobile phone ownership among youth provides an opportunity to leverage mobile health applications (app) to increase HIV/STI testing and PrEP uptake among YMSM. Obje...
