چکیده

eart failure begets mitral regurgitation (MR); MR begets eart failure. This incestuous and insidious relationship has ong been known to contribute to the positive feedback loop hat underlies progressive, pathologic ventricular remodelng in chronic heart failure (CHF). Yet, despite decades of nvestigation and thought, important questions have gone nanswered, many with direct relevance to the care of CHF atients with MR. Fundamentally, these questions revolve round understanding how much MR contributes to sympoms and disease progression, whether to intervene and, if o, when to intervene surgically to eliminate MR. With lack f randomized clinical trials proving benefit, along with ssociated risks, surgical correction of MR is not performed

Surgical management of functional mitral regurgitation in heart failure patients

Relevant secondary mitral regurgitation occurs in 35– 50% of patients with chronic heart failure and is associated with increased mortality. Surgical management of patients with mitral valve regurgitation and heart failure remains controversial. There are several questions which are not sufficiently clarified because of a lack of prospective randomised controlled trials: (1.) The indication for...

Evaluation of diastolic blood flow dynamic of the left ventricle in dogs with mitral valve regurgitation using vector flow mapping

Mitral valve regurgitation (MVR) is a common valvular disease in dogs. Hydrokinetic evaluation of the blood flow within the ventricles has become possible by vector flow mapping (VFM), which shows the blood flow within the ventricles in vector and vortex flows. Blood flow within the left ventricle of MVR dogs was compared at different stages of MVR and to that of normal dogs. 14 normal dogs and...

اثر PercutaneousCoronary Intervention Elective بر بهبودی نارسایی ایسکمیک میترال

Introdution: Ischemic mitral regurgitation (IMR) remains one of the most complex and unresolved aspects of ischemic heart disease that the impact of percutaneous coronary intervention (PCI) on improvement of intensity of ischemic mitral regurgitation is not well clarified. Patients with coronary artery diseases and ischemic mitral regurgitation have a worse prognosis than the patients with coro...

The impact of baseline left ventricular size and mitral regurgitation on reverse left ventricular remodelling in response to carvedilol: size doesn't matter.

b Blockers have been shown to reverse left ventricular remodelling (LVR) and to reduce the severity of functional mitral regurgitation (FMR) in patients with chronic systolic heart failure. It is unclear, however, to what extent the beneficial effects of b blockers on LVR or FMR are influenced by the severity of left ventricular dilatation or FMR at the time of commencement of b blocker treatme...

Doing away with dogma: increasing afterload to reduce mitral regurgitation.

As heart failure due to coronary and other nonvalvular heart disease becomes more prevalent in the urbanized world, functional mitral regurgitation will become a problem of increasing importance (1). Functional mitral regurgitation, which decreases effort tolerance and worsens prognosis in cardiomyopathy (2,3), is present in most patients referred for cardiac transplantation (4). It is therefor...
