The Lower Limit of Cbf Autoregulation

Copyright © 2013 International Anesthesia Research Society PERSPECTIVE I have committed considerable professional time to anesthesia for neurologic surgery. That commitment has caused me to hear a great deal related to neuroanesthesia. I have been asked by faculty colleagues in anesthesia, “What do you suppose might have gone wrong during that case yesterday?” I have listened to American Board of Anesthesiology oral examination candidates misapplying or frankly misunderstanding fundamental principles. I have been involved in third-party scrutiny of procedures that were perceived to have gone astray. I acknowledge that those interactions inform the selection of topics that follows (rather than “evidence-based” material from the peer-reviewed literature). Not all of the topics will be addressed during the course of the presentation.

Bradykinin mediates the acute effect of an angiotensin-converting enzyme inhibitor on cerebral autoregulation in rats.
BACKGROUND AND PURPOSE In patients with stroke and long-standing hypertension, the autoregulation curve of cerebral blood flow (CBF) shifts toward higher blood pressure levels. Angiotensin-converting enzyme (ACE) inhibitors reduce blood pressure and shift the autoregulation curve back to normal in hypertensive patients. ACE inhibitors have 2 major pharmacological properties: they inhibit both t...

BACKGROUND AND PURPOSE This study validates transcranial Doppler sonography (TCD) for determination of the lower limit of cerebral blood flow (CBF) autoregulation and establishes a relation between global CBF and mean flow velocity (Vmean) in the middle cerebral artery. METHODS Relative changes in CBF and in Vmean were compared in 12 normal volunteers (2 women and 10 men; median age, 30 years...
متن کاملNo effect of angiotensin II AT(2)-receptor antagonist PD 123319 on cerebral blood flow autoregulation.
Blockade of the renin-angiotensin system with angiotensin-converting enzyme inhibitors (ACE-I) or angiotensin AT1-receptor antagonists shift the limits of autoregulation of cerebral blood flow (CBF) towards lower blood pressure (BP). The role of AT2-receptors in the regulation of the cerebral circulation is uncertain. Hence, the present study investigated the effect on CBF autoregulation of blo...
متن کاملAutoregulation of cerebral blood flow in hypertensive patients. The modifying influence of prolonged antihypertensive treatment on the tolerance to acute, drug-induced hypotension.
Autoregulation of cerebral blood flow (CBF) was studied by the arteriovenous oxygen difference method in 13 patients with untreated or ineffectively treated severe hypertension, nine patients with effectively treated, formerly severe hypertension, and ten normotensive controls. Resting mean blood pressure in these three groups was 145 +/- 17 (1 SD) mm Hg, 116 +/- 18 mm Hg, and 98 +/- 10 mm Hg, ...
متن کاملEffect of nephrectomy and captopril on autoregulation of cerebral blood flow in rats.
The present study investigated the effect of circulating versus locally present renin on cerebral blood flow (CBF) and its autoregulation in rats. CBF was measured repetitively with the intracarotid 133Xe injection method, whereas blood pressure was lowered to determine the lower limit of autoregulation. To remove renin from the blood, rats were bilaterally nephrectomized and kept alive with pe...
متن کاملChronic antihypertensive treatment in the rat reverses hypertension-induced changes in cerebral blood flow autoregulation.
Cerebral blood flow (CBF) autoregulation was studied in renal hypertensive rats receiving chronic antihypertensive treatment. Young Wistar Kyoto rats (WKY) were made hypertensive by the Loomis procedure i.e. partial infarction of one kidney with contralateral nephrectomy. Systolic tail blood pressure was measured at 2-week intervals throughout the study. After two months, by which time the rats...
