چکیده

In atrial fibrillation (AF), the loss of coordinated electromechanical atrial activity predisposes to impaired atrial emptying, stasis of blood, and a prothrombotic state.1 These factors cause blood clots to form in the heart and embolize systemically to cause stroke or other organ failure.2,3 AF is responsible for 15% of 700 000 strokes in the United States each year, with an estimated annual direct and indirect cost of $57.9 billion in the United States alone.4

European Primary Care Cardiovascular Society (EPCCS) consensus guidance on stroke prevention in atrial fibrillation (SPAF) in primary care.

BACKGROUND Atrial fibrillation affects 1-2% of the general population and 10% of those over 75, and is responsible for around a quarter of all strokes. These strokes are largely preventable by the use of anticoagulation therapy, although many eligible patients are not treated. Recent large clinical trials have added to the evidence base on stroke prevention and international clinical guidelines...

Benchmarks for acute stroke care delivery

OBJECTIVE Despite widespread interest in many jurisdictions in monitoring and improving the quality of stroke care delivery, benchmarks for most stroke performance indicators have not been established. The objective of this study was to develop data-derived benchmarks for acute stroke quality indicators. DESIGN Nine key acute stroke quality indicators were selected from the Canadian Stroke Be...

Improving stroke prevention therapy for patients with atrial fibrillation in primary care: protocol for a pragmatic, cluster-randomized trial

BACKGROUND The prevalence of atrial fibrillation (AF) is growing as the population ages, and at least 15% of ischemic strokes are attributed to AF. However, many high-risk AF patients are not offered guideline-recommended stroke prevention therapy due to a variety of system, provider, and patient-level barriers. METHODS We will conduct a pragmatic, cluster-randomized controlled trial randomiz...

He suffered a multiple bone fracture with chief complaints of pain and shortness of breath. He was transfer...

New oral anticoagulants in atrial fibrillation.

Atrial fibrillation (AF) is a major risk factor for stroke. Currently, acetylsalicylic acid (a platelet inhibitor) and vitamin K antagonists (VKAs; oral anticoagulants), including warfarin, are the only approved antithrombotic therapies for stroke prevention in patients with AF. Although effective, VKAs have unpredictable pharmacological effects, requiring regular coagulation monitoring and dos...
