Functional Knee Outcomes in Suprapatellar and Infrapatellar Tibial Nailing: Does Approach Matter?
نویسندگان
چکیده
Introduction With an incidence of 75,000 per year in the United States alone, fractures of the tibial shaft are among the most common long bone fractures. Techniques using a semi-extended suprapatellar approach can facilitate intraoperative imaging, allow easier access to starting site position and counter deforming forces. While outcomes following traditional infrapatellar nailing have been well documented, there is a paucity of literature regarding outcomes following the use of a suprapatellar approach. By splitting the quadriceps tendon, scar tissue will form superior to the patella as opposed to the anterior knee, which may reduce flexion-related pain or pain while kneeling. The infrapatellar nerve is also well protected during this approach. The purpose of this study was to determine differences in functional knee pain in patients who underwent suprapatellar nailing versus traditional infrapatellar nailing.

Comparison of insertional trauma between suprapatellar and infrapatellar portals for tibial nailing.
The purpose of this study was to determine differences in insertional articular trauma in infrapatellar tibial portal and suprapatellar portal intramedullary tibial nail insertion techniques. A cadaveric study was performed on 10 matched pairs of fresh-frozen adult cadaver lower extremities with intact extensor mechanisms. Two study groups with 10 limbs each were created: left lower limbs were ...

can be difficult when using the standard entry portal. We evaluated the suprapatellar portal, using a midline quadriceps tendon incision, to perform intramedullary nailing of the tibia. Seven patients were treated with this adaptation of the standard intramedullary nailing procedure. An arthroscopy was done before and after the nailing procedure. No special equipment was used to perform the int...

Reamed locked intramedullary nailing remains the standard treatment for displaced tibial shaft fractures. Supra-patellar tibial nailing in the semiextended position has been suggested as a safe and effective surgical technique that allows mitigating certain challenges of the standard subpatellar approach. Suprapatellar nailing seems to facilitate achieving and maintaining fracture reduction, pa...
متن کاملUse of the Suprapatellar Approach in Intramedullary Nailing of a Multi-Fragmentary Dislocated Tibia Fracture with a Hypermobile Intermediate Fragment in a Young Patient
A case of an adolescent female patient who suffered from first grade open multi-fragment fracture of the tibia (AO42-C2) with a large hypermobile intermediate fragment is presented in this case report. Intramedullary nailing of the tibia remains the treatment of choice despite a high risk of malformation and anterior knee pain especially in multi-fragment fractures. Here the suprapatellar appro...
متن کاملSuprapatellar nailing of tibial fractures: surgical hints
Intramedullary nailing of the tibia with suprapatellar entry and semi-extended positioning makes it technically easier to nail the proximal and distal fractures. The purpose of this article was to describe a simple method for suprapatellar nailing (SPN). A step-by-step run through of the surgical technique is described, including positioning of the patient. There are as yet only a few clinical ...
