T-wave oversensing during left ventricular pacing causing inappropriate defibrillation

چکیده
Introduction Cardiac resynchronization therapy (CRT) with left ventricular (LV)-only pacing has emerged as an alternative to biventricular (Bi-V) pacing used to synchronize ventricular contraction by timing LV pacing with intrinsic right ventricular (RV) conduction. Additionally, RV pacing is minimized, improving device longevity. Despite its benefits, LV pacing may not the best modality for all patients. We present a case of T-wave oversensing (TWOS) during LV-only pacing using a dynamic, proprietary algorithm resulting in diminishedventricular pacing and inappropriate defibrillation. This resolved with reprogramming to Bi-V pacing, providing a solution to a unique problem.

Hyperglycemia-Induced T-Wave Oversensing as a Cause of Cardiac Resynchronization Therapy (CRT) Failure
T-wave oversensing occurs when the counter starts giving dual beeps for every cardiac cycle instead of one. This usually happens when the monitoring lead displays a tall T wave, which is also sharp. R wave sensing algorithms of the devices do not sense T wave because the slow rate of the T wave is much less than that of the R wave. But the slow rate of T waves may change with time and also beca...

We describe the case of a patient who presented with multiple implantable cardioverter-defibrillator (ICD) shock discharges 12 months after device implantation. Upon device interrogation, intermittent oversensing of electrical noise and potential ICD lead failure were suggested by a significant increase in the sensing integrity counter (SIC), a cumulative count of very short ventricular sensed ...
متن کاملAuthors Reply to 'Strategies for Overcoming T-Wave Oversensing'
In their letter to the editor Cay et al [1] express some valid concerns regarding the strategy we described for overcoming T-wave oversensing in our patient [2]. They stress that programmable features of the device should be used first before planning a reintervention. As mentioned in our case-report we tried both the enhanced T wave suppression algorithm available in the device and programming...
متن کاملT-wave Oversensing with Inappropriate Therapy in Remote Monitoring
T-wave oversensing can cause inappropriate implantable cardioverter-defibrillator (ICD) therapies that are difficult to correct. Remote monitoring allows follow-up of ICD patients without visiting the hospital and can help in early detection of any malfunctions. We describe the case of a patient who experienced inappropriate antitachycardia pacing therapy due to T-wave oversensing; the problem ...
