Secondary nipple reconstruction using local flaps and AlloDerm.

چکیده
Over the years, a variety of techniques to reconstruct the nipple following mastectomy and reconstruction have been described. The most commonly used techniques involve the use of local flaps that include the star, bell, skate, double-opposing tab, and C-V.1–3 All of these methods require a series of incisions on the breast mound, creation of local flaps with random blood supply, and rearrangement of these flaps to reconstruct the nipple. Although the initial projection is excellent, all will flatten to some degree over time. Shestak et al. have compared the skate, star, and bell flaps and demonstrated flattening by 30 to 75 percent within 2 years.1 Losken et al. have analyzed long-term nipple projection using the C-V flap and demonstrated an average projection of 3.77 mm with a patient satisfaction of 42 percent at an average follow-up of 5.3 years.2 Kroll et al. have compared the double-opposing tab and star flaps and found that the mean 2-year projection was 2.42 mm and 1.97 mm, respectively.3 In a survey assessing patient satisfaction following nipple reconstruction, Jabor et al. have demonstrated that excessive flattening of the nipple was the principal area of dissatisfaction in over 50 percent of the women questioned.4 To overcome the tendency of reconstructed nipples to flatten, a variety of materials have been used to augment nipple projection that includes subcutaneous fat, bone, cartilage, and silicone.5–8 These materials have been used for primary and secondary nipple reconstruction and sometimes result in improved long-term projection; however, a disadvantage of these methods is that they rely on foreign material or autologous material harvested from remote areas of the body. This can occasionally result in donor-site morbidity such as scars, pain, and distortion. AlloDerm (LifeCell Corporation, Branchburg, N.J.) is an acellular dermal graft that has been demonstrated to revascularize and become incorporated into recipient tissues with minimal morbidity.9,10 It has been successfully used in a variety of clinical and experimental settings9,11,12; however, its use to increase projection of the nipple following reconstruction has not been previously described. The purpose of this study was to review my initial experience and outcome with AlloDerm for secondary and tertiary nipple reconstruction.

Use of AlloDerm in primary nipple reconstruction to improve long-term nipple projection.
BACKGROUND The objective of this study was to demonstrate the use of the authors' technique to improve long-term maintenance of nipple projection by using AlloDerm (LifeCell Corp., Branchburg, N.J.) as a central core in nipple reconstruction. METHODS The nipple reconstruction technique involved the use of a modified star dermal flap pattern measuring 5 cm in length and 1.0 to 1.5 cm in width,...
متن کاملBreast Reconstruction Using Contour Fenestrated AlloDerm: Does Improvement in Design Translate to Improved Outcomes?
BACKGROUND Acellular dermal matrices are used in implant-based breast reconstruction. The introduction of contour fenestrated AlloDerm (Life-Cell, Branchburg, N.J.) offers sterile processing, a crescent shape, and prefabricated fenestrations. However, any evidence comparing reconstructive outcomes between this newer generation acellular dermal matrices and earlier versions is lacking. METHODS...
متن کاملA Novel Nipple Reconstruction Technique for Maintaining Nipple Projection: The Boomerang Flap
Nipple-areolar complex (NAC) reconstruction is the final step in the long journey of breast reconstruction for mastectomy patients. Successful NAC reconstruction depends on the use of appropriate surgical techniques that are simple and reliable. To date, numerous techniques have been used for nipple reconstruction, including contralateral nipple sharing and various local flaps. Recently, it has...
متن کاملA Rapid, Simple, Effective, and Inexpensive Reconstructed Nipple Flap Guard
Nipple reconstruction is a commonly performed component of breast reconstruction. A nipple reconstructed using local skin flaps requires protection from trauma. Here we describe a novel, effective, simple, rapid, inexpensive, and convenient method to protect a reconstructed nipple in the early postoperative period.
متن کاملA New Local Flap Nipple Reconstruction Technique Using Dermal Bridge and Preoperatively Designed Tattoo
BACKGROUND Nipple-areolar reconstruction is the final step in breast reconstruction. Reconstruction using local flaps and tattooing is useful in cases of bilateral reconstruction, a small nipple-areolar complex (NAC) as the donor site, and avoiding disturbance of the normal side and other body parts. However, this method can cause projection loss and color fading of the nipple. Moreover, the br...
