A 62-year-old woman who had suffered from dyspnoea on exertion was referred to our hospital. She had no past or family history of medical illnesses. Chest X-ray showed mild cardiomegaly. Serum cre-atinine and brain natriuretic peptide level were mildly elevated as 1.66 mg/dL and 1,418 ng/L, respectively. Although left ventricular (LV) ejection fraction was preserved, thickened LV myocardium with a restrictive LV filling pattern, suggesting restrictive cardiomy-opathy, was noted on echocardiography (Panel A and see Supplementary data online, Video S1). Notably, tricuspid valve was thickened and stenotic, accompanied by mitral stenosis, which could be usually seen in rheumatic heart disease (Panels B–D and see Supplementary data online, Video S2). T 2-weighted magnetic resonance imaging showed no LV myocardial oedema. Subendocardial and diffuse transmural late gadolinium enhancement and a marked expansion of extracellular space on pre-and post-T 1 maps, suggesting cardiac amyloidosis, were shown (Panels E and F and see Supplementary data online, Video S3). A cardiac biopsy showed Congo red-positive with apple-green birefringence under polarized light (Panel G). Finally, under the diagnosis of cardiac amyloidosis with bi-valvular involvement, she was treated with melphalan/dexamethasone chemotherapy. Tricuspid stenosis is uncommon in adults, and in most cases, it is the result of rheumatic heart disease with rheumatic mitral involvement. Amyloidosis involving a significantly stenotic tricuspid valve is also extremely rare. Without careful cardiac imaging and systemic evaluation, this case could have been misdiagnosed as rheumatic valvular heart disease involving the mitral and tricuspid valves.

Isolated tricuspid valve stenosis--probably of rheumatic etiology.
Rheumatic heart disease is still a common cardiac disease in developing countries. Triscupid valve is not usually involved in RHD and whenever it is involved, it is always associated with mitral valve disease. We report a case of isolated tricuspid stenosis, most likely of rheumatic etiology.
متن کاملThe Opening Snap of the Tricuspid Valve : A Physical Sign of Tricuspid Stenosis
In two patients with rheumatic heart disease a short, loud, high pitched, early diastolic, snapping sound was heard at or below and to the right of the xiphisternum. Except for the different area of audibility it was similar on auscultation to the opening snap of the mitral valve. Both patients showed clinical evidence of tricuspid as well as mitral and aortic valvular disease. There was hemody...
متن کاملUnusual Giant Right Atrium in Rheumatic Mitral Stenosis and Tricuspid Insufficiency
Dilation and hypertrophy of the atria occur in patients with valvular heart disease especially in mitral regurgitation, mitral stenosis or tricuspid abnormalities. In sub-saharan Africa, rheumatic fever is still the leading cause of valvular heart disease. We report a case of an unusual giant right atrium in context of rheumatic stenosis and severe tricuspid regurgitation in a 58-year-old woman.
متن کاملThe value of left heart catheterization in patients with rheumatic mitral valve disease.
Diagnostic and therapeutic problems commonly arise in patients with rheumatic heart disease despite the use of all available clinical data, including electrocardiographic and roentgenographic studies. The deficiencies of the conventional aids have become more apparent with attempts to define the physiological significance of mitral valve lesions. Since its introduction by Cournand and associate...
متن کاملRheumatic aortic stenosis in young patients presenting with combined aortic and mitral stenosis.
This report describes 30 patients under the age of 30 years with rheumatic aortic stenosis, presenting with combined aortic and mitral stenosis. Three patients had additional tricuspid stenosis. Twenty-eight patients gave a history of rheumatic polyarthritis. The diagnosis was confirmed by right and left heart catheterisation in all. The murmur of aortic stenosis was not initially present in 8 ...
