Mitral valve apparatus preservation and early bioprosthetic thrombosis: a word of caution.

چکیده
Mitral valve/surgery; thrombosis; bioprosthesis. (prosthesis M-25, BSA = 1,672 m2) and 6 mmHg (prosthesis M-27, BSA = 1,632 m2), respectively. Neither of the patients was receiving anticoagulants. The case of the youngest patient was more dramatic, because she was admitted to the emergency room with history suggesting paroxysmal acute atrial fibrillation and neurological signs of embolization 18 days after surgery. The transesophageal echocardiogram (TEE) revealed thrombosis affecting the left auricle, atrial wall, and atrial face of the prosthesis. These data were confirmed at the emergency reoperation, when a new mechanical prosthesis was implanted with resection of the leaflets and annular fixation of the papillary muscles.

Early bioprosthetic valve failure caused by preserved native mitral valve leaflets.
The importance of preservation of subvalvular apparatus and valve-ventricular continuity during mitral valve replacement (MVR) has been suggested for many years. The chordal-sparing MVR has been shown to be superior to the standard MVR with chordal resection in terms of improved left ventricular function and has been considered to be a safe procedure. However, we encounter a rare case requiring...
متن کاملAcute thrombosis of bioprosthetic mitral valve
We report a case of acute thrombosis of bioprosthetic mitral valve in a 59 year-old Korean female, who underwent a mitral valve replacement with a 25 mm Carpentier - Edwards PERIMOUNT Plus bioprosthesis (Edwards Lifesciences, Inc.; Irvine, CA, USA) and a mini-Maze procedure for correction of mitral stenosis (MS) and atrial fibrillation (AF). On the 10th postoperative day, the patient began to c...
متن کاملUnexpected bioprosthetic mitral valve thrombus during left ventricular assist device implantation
Acute bioprosthetic valve thrombosis can occur after surgery and sometimes cause hemodynamic instability and cardiogenic shock. Risk factors for bioprosthetic valve thrombosis are hypercoagulability, atrial fibrillation, atrial dilatation, low cardiac function, and lack of anticoagulation therapy. The authors present a case of severe mitral stenosis due to bioprosthetic valve thrombus. The pati...
متن کاملBioprosthetic mitral valve thrombosis less than one year after replacement and an ablative MAZE procedure: a case report
Occurrence of bioprosthetic valve thrombosis less than a year after replacement is very uncommon. Here, we describe a case of a 57 year old male, who presented 10 months after receiving a bioprosthetic mitral valve replacement with a two week history of dyspnea on exertion, worsening orthopnea and decreased exercise tolerance. Echocardiography revealed severe mitral regurgitation (MR), thrombos...
متن کاملBioprosthetic mitral valve thrombosis. Importance of transesophageal echocardiography in the diagnosis and follow-up after treatment.
OBJECTIVE To report the clinical and echocardiographic findings of bioprosthetic mitral valve thrombosis and the value of transesophageal echocardiography (TEE) in its diagnosis and monitoring of thrombolysis. METHODS One hundred and eleven patients with mitral bioprostheses underwent TEE, and 4 out of 7 suspected of having a thrombus on these prostheses were included in the study (mean age =...
