How can we achieve better prevention of progression to tuberculosis among contacts?

چکیده
Strategies for control and elimination of tuberculosis (TB) in low-incidence settings are directed toward treatment of recently acquired latent tuberculosis infection (LTBI) in TB contacts [1]. To identify this target population for preventive treatment the development of more specific, in vitro assays for LTBI, the interferon (INF)-c release assays (IGRAs), has offered an alternative method for LTBI diagnosis. Although IGRAs are increasingly recommended in national guidelines, evidence that positive IGRA results are prognostic for developing TB is still limited [2, 3], especially outside of prospective studies with welldefined inclusion criteria. Therefore, we investigated progression towards active TB among IGRA positive contacts of active TB cases under routine field conditions and calculated the positive predictive value (PPV) for progression and the number needed to treat (NNT) with preventive treatment to prevent one incident TB case. Additionally, we introduced different cut-off values for IGRA positivity and compared the computed progression rates. Among all contacts with a positive tuberculosis-specific IGRA we describe the uptake of preventive treatment.

Attitudes about Tuberculosis Prevention in the Elimination Phase: A Survey among Physicians in Germany
BACKGROUND Targeted and stringent measures of tuberculosis prevention are necessary to achieve the goal of tuberculosis elimination in countries of low tuberculosis incidence. METHODS We ascertained the knowledge about tuberculosis risk factors and stringency of tuberculosis prevention measures by a standardized questionnaire among physicians in Germany involved in the care of individuals fro...
متن کاملKnowledge about tuberculosis transmission and prevention and perceptions of health service utilization among index cases and contacts in Brazil: Understanding losses in the latent tuberculosis cascade of care
INTRODUCTION Tuberculosis contacts are candidates for active and latent tuberculosis infection screening and eventual treatment. However, many losses occur in the different steps of the contacts' cascade of care. Reasons for this are poorly understood. OBJECTIVE To describe the different steps where losses in the contact cascade occur and to explore knowledge and attitudes regarding tuberculo...
متن کاملTuberculosis Contact Investigations--United States, 2003-2012.
Mycobacterium tuberculosis is transmitted through the air from an infectious patient (index patient) to other persons (contacts) who share space. Exposure to M. tuberculosis can result in tuberculosis (TB) disease or latent TB infection (LTBI), which has no clinical symptoms or radiologic evidence of disease. The cycle of transmission can be ended by isolating and treating patients with TB dise...
متن کاملInvestigation of Unprotected Effective Contact of Tuberculosis Patients in Their Families in Kerman
Background and Objectives: At present, investigation of extensive contact of tuberculosis (TB) patients’ families is not a routine part of TB control efforts in most countries. This study was conducted to evaluate the effective contacts of TB cases to assess the potential risk of transmission and its determinants in their families. Methods: In this cross-sectional study, registered TB ca...
متن کاملPreventing the spread of multidrug-resistant tuberculosis and protecting contacts of infectious cases.
Prevention of multidrug-resistant and extensively drug-resistant tuberculosis (MDR/XDR-TB) is a top priority for global TB control, given the need to limit epidemic spread and considering the high cost, toxicity and poor treatment outcomes with available therapies. We performed a systematic literature review to evaluate the evidence for strategies to reduce MDR/XDR-TB transmission and disease p...
