Governments should reduce prices before rationing care.

چکیده
I welcome the Comment by Didier Raoult (Jan 9, p 104), which proposes the post-Darwinist concept of the evolutionary pathway as a rhizome instead of a tree, with inheritance occurring not only vertically but also laterally. He rightly addresses how the genomic revolution has forced us to reconsider the way we describe evolution. Perhaps one of the most important unresolved issues in the study of species evolution is the adaptation to a changing environment, bringing all the evolutionary changes seen in a short period. Epigenetics could explain this phenomenon in eukaryotic species including human beings. Most epigenetic changes occur within the course of one individual organism’s lifetime, but, in recent investigations, epigenetic changes were found to be inherited from one generation to the next: the inheritance of acquired characteristics of Lamarck. Of course, Lamarckism’s examples of giraffes stretching their necks to reach leaves high in trees, gradually lengthening their necks, and then having offspring with slightly longer necks only shows that generational change is not a simple thing. However, it has been shown that the epigenetic marks by DNA methylation can be inherited across multiple generations in mice, implying that not only prenatal maternal but also grandmaternal exposures to methyl donors may influence subsequent gene expression in the offspring. I propose that our current understanding of epigenetics could partly reinstate Lamarck in the light of a post-Darwinist model of evolution.

Rationing Poor Consumers to Reduce Prices
We study how rationing in the public sector influences prices in the private sector. A private firm uses consumers’ cost information for cream-skimming. Only rationed consumers consider purchasing from the private firm. Rich consumers are more willing to pay for an indivisible good, such as a health treatment, than poor consumers. The public supplier decides on a rationing rule, and then the pr...
متن کاملRationing in Medicine: A Presupposition for Humanity and Justice
Limited resources are the permanent condition in health care. Rationing, according to H. Kliemt, is the distribution of limited resources below market prices to all people in need for these resources. Therefore, rationing is a basic component of every kind of human health care system. However, the crucial problem is how to find just and fair rules for this distribution under the premise, that e...
متن کاملPublic participation and the moral quality of healthcare rationing.
Health services throughout the world do not have enough resources to meet the healthcare needs of all of their citizens. Therefore, some form of rationing within health services is inevitable. Even within healthcare systems in which access to health care is guaranteed— however they are funded—this will mean that some who are ill will not necessarily receive treatment when they need it, if at al...
متن کاملIndustry equilibrium with outside #nancing and moral hazard: Implications for market integration
In this paper, we study industry equilibrium under the assumptions that (1) #rms need outside #nancing and (2) they have a moral hazard problem in taking potentially excessive risks. We characterize an industry equilibrium with credit rationing, where #rms choose not to take risks, and compare this to the industry equilibrium in the absence of credit rationing. In both cases, we show that compe...

mony is a virtue to which all physicians should strive in the practice of medicine. However, we disagree with the statement in the article that “Both parsimonious medicine and rationing aim to reduce resource use.” The aim of medical parsimony is to provide the care necessary for the patient’s good, not to reduce resource use (although it may in addition preserve resources)—a difference in inte...
