چکیده

©2015 by The Orthopaedic Journal at Harvard Medical School BACKGROUND Shoulder arthroplasty procedures have generally excellent clinical outcomes, but infection following shoulder arthroplasty continues to be a difficult problem both diagnostically and therapeutically. Two-stage reimplantation with use of a temporary Prosthesis of Antibiotic-Loaded Acrylic Cement (PROSTALAC) (DePuy, Warsaw, Indiana) is the standard treatment for chronic infection. Optimizing surgical technique regarding cement spacer is a priority, but detailed technical descriptions are sparse.

The PROSTALAC functional spacer in two-stage revision for infected knee replacements. Prosthesis of antibiotic-loaded acrylic cement.

The PROSTALAC functional spacer is made of antibiotic-loaded acrylic cement but has a small metal-on-polythene articular surface. We have used it as an interim spacer in two-stage exchange arthroplasty for infected total knee replacement. PROSTALAC allows continuous rehabilitation between stages as it maintains good alignment and stability of the knee and a reasonable range of movement. It also...

Treatment of an infected total hip replacement with the PROSTALAC system. Part 1: Infection resolution.

INTRODUCTION Infection after total hip replacement (THR) is a serious medical complication with significant negative ramifications for both the patient and the health care system. The prosthesis of antibiotic-loaded acrylic cement (PROSTALAC) was designed to treat the joint infection while maintaining functional movement in the hip. METHODS We identified 28 patients treated for infected THR w...

Treatment of an infected total hip replacement with the PROSTALAC system. Part 2: Health-related quality of life and function with the PROSTALAC implant in situ.

INTRODUCTION Infection after total hip replacement (THR) adversely affects patients' function and health-related quality of life (HRQL). A prosthesis with antibiotic-loaded acrylic cement (PROSTALAC) was designed to improve the function and quality of life of patients undergoing treatment for infected THR. METHODS We assessed 23 patients with the PROSTALAC implant in situ for treatment of an ...

Antibiotic-loaded bone cement reduces deep infection rates for primary reverse total shoulder arthroplasty: a retrospective, cohort study of 501 shoulders.

BACKGROUND Deep infection after primary reverse total shoulder arthroplasty is a devastating event and has an increased incidence compared with anatomic total shoulder arthroplasty. Recent reports in the hip and knee arthroplasty literature suggest that antibiotic-loaded bone cement may lower infection rates for primary arthroplasties. We conducted a retrospective cohort study to evaluate the e...

Antibiotic-loaded bone cement for infection prophylaxis in total joint replacement.

Use of antibiotic-loaded bone cement for prophylaxis against infection is not indicated for patients not at high risk for infection who are undergoing routine primary or revision joint replacement with cement. The mechanical and elution properties of commercially available premixed antibiotic-loaded bone-cement products are superior to those of hand-mixed preparations. Use of commercially avail...
