Stereotactic thalamotomy and L-dopa induced involuntary movement in Parkinsonism.

چکیده
Involuntary movement is one of the most disturbing and problematic side effects of L-dopa in the treatment of Parkinsonism. Several attempts have been made to reduce this complication, such as an addition of adjuvant, (4) and use of slow release L-dopa. However, the problem remains unsolved probably because hyperkinesis occurs when the akinesis of Parkinonism is overcorrected by L-dopa and produces hyperkinesis (15). For more than 15 years we have been treating Parkinsonian patients with stereo tactic thalamotomy or pallidotomy or both and began to use L-dopa since 1968. We found that the previous operation reduces the incidence and severity of L-dopa in duced involuntary movements in the treated limbs suggested by several authors (1) (15) (17) (20). This paper is an analysis of L-dopa induced involuntary movements seen in 29 cases in 121 Parkinsonian patients treated with L-dopa, 94 cases with the previous stereotactic procedures and 27 non-surgical cases.

L-dopa in Parkinsonism and the influence of previous thalamotomy.
A double-blind cross-over trial over 24 weeks (10 weeks on the active remedy, 4 weeks off treatment, and 10 weeks on placebo) of the effect of L-dopa on idiopathic Parkinsonism (paralysis agitans) has shown no difference in the response obtained in patients who had undergone previous stereotaxic ventrolateral thalamotomy and in those who had not. Of the 34 patients (18 men and 16 women) in the ...
متن کاملGastrodia elata Blume alleviates L-DOPA-induced dyskinesia by normalizing FosB and ERK activation in a 6-OHDA-lesioned Parkinson’s disease mouse model
BACKGROUND Gastrodia elata Blume (GEB), commonly used medicinal herb, has been reported as a promising candidate for neurodegenerative diseases such as Parkinson's disease. The dopamine precursor, L-3,4-dihydroxyphenylalanine (L-DOPA), is the gold-standard drug for Parkinson's disease, but long-term treatment results in the L-dopa-induced dyskinesia (LID). This study was undertaken to examine t...
متن کاملLack of tolerance for the anti-dyskinetic effects of 7-nitroindazole, a neuronal nitric oxide synthase inhibitor, in rats.
7-Nitroindazole (7-NI) inhibits neuronal nitric oxide synthase in vivo and reduces l-DOPA-induced dyskinesias in a rat model of parkinsonism. The aim of the present study was to determine if the anti-dyskinetic effect of 7-NI was subject to tolerance after repeated treatment and if this drug could interfere with the priming effect of l-DOPA. Adult male Wistar rats (200-250 g) with unilateral de...
متن کاملDrug-induced dyskinesia--an electrophysiological analysis of dyskinesia induced by L-DOPA and anticholinergic drugs.
The application of L·DOPA (L-3, 4-dihydroxyphenylalanine) to the treatment of extrapyramidal disorders has been extremely beneficial for parkinsonism 1)-8). At the same time several side-effects have been recognized. Among them, the appearance of dyskinesia poses an important problem 8)-13l , but causative mechanisms of L·DOPA induced dyskinesia are not yet sufficiently elucidated. Such dyskine...
متن کاملFrequency analysis of involuntary movements during wrist tracking: a way to identify ms patients with tremor who benefit from thalamotomy.
To identify those multiple sclerosis (MS) patients with disabling tremor who will benefit most from thalamotomy, measurements of frequency spectra of involuntary movements during visually guided wrist tracking were carried out in 11 consecutive patients with MS before and after ventrolateral thalamotomy. Thalamotomy was significantly more effective if patients had disruptive action tremor which...
