Computerized Dynamic Posturography (CDP) and the Assessment of Balance with Active Head Movements

چکیده

Of the numerous tools available for assessing patients with imbalance, the most clinically effective one is an accurate clinical history i.e., the patient’s past and presenting symptoms. Once the symptoms have been established, then one can select the appropriate laboratory studies which when interpreted in conjunction with that history should offer information relative to site-of-lesion, narrow the etiological possibilities, and provide information relative to the functional impact of the problem. The functional information provided by CDP plays a key role in contributing to treatment design, particularly to a Vestibular and Balance Rehabilitation Therapy (VBRT) protocol and in monitoring the patient’s progress. In fact, when Stewart and colleagues 24) examined the cost effectiveness of the diagnostic evaluation of the dizzy patient within the context of this model, CDP proved to be the most cost-effective test of balance. This finding is enhanced by the role of CDP in differentiating between true and exaggerated abnormal symptoms. Other prospective studies involving patients with idiopathic Parkinson’s disease have shown that CDP impairment information can be of significant value in identifying patients likely to benefit from surgical, deep brain stimulation, and medication treatments.

Destabilization of human balance control by static and dynamic head tilts.

To better understand the effects of varying head movement frequencies on human balance control, 12 healthy adult humans were studied during static and dynamic (0.14, 0.33, 0.6 Hz) head tilts of +/- 30 degrees in the pitch and roll planes. Postural sway was measured during upright stance with eyes closed and altered somatosensory inputs provided by a computerized dynamic posturography (CDP) syst...

Abnormal computerized dynamic posturography findings in dizzy patients with normal ENG results.

The complexities of the balance system create difficulties for professionals interested in testing equilibrium function objectively. Traditionally, electronystagmography (ENG) has been used for this purpose, but it provides information on only a limited portion of the equilibrium system. Computerized dynamic posturography (CDP) is less specific than ENG, but it provides more global insight into...

Misclassification of Patients with Spinocerebellar Ataxia as Having Psychogenic Postural Instability Based on Computerized Dynamic Posturography

Specific criteria have been developed based on computerized dynamic posturography (CDP) to assist clinicians in identifying patients with psychogenic balance problems (Cyr and Cevette, 1993; Cevette et al., 1995; Goebel et al., 1997; Gianoli et al., 2000). Patients with known spinocerebellar ataxia (SCA) meet several of the criteria for psychogenic balance problem and risk being misclassified a...

Modified head shake computerized dynamic posturography.

PURPOSE Recent research on head shake posturography has demonstrated a modest increase in sensitivity to identifying peripheral vestibular system asymmetry when horizontal head movements were added to portions of the standard Sensory Organization Test (SOT) battery. However, limitations with respect to the head shake protocol were outlined, and usable data for assessing performance could not be...

Balance as a measurement of fatigue in postcall residents.

OBJECTIVES/HYPOTHESIS To determine the degree to which balance is impaired by fatigue in postcall medical residents and to assess the validity of posturography in quantifying that impairment. STUDY DESIGN Prospective study. METHODS Medical residents underwent the following tests before and immediately after overnight call: 1) computerized dynamic posturography (CDP) using the Equitest force...
