Case scenario: postoperative liver failure after liver resection in a cirrhotic patient.

چکیده
H EPATIC resection for malignant liver tumors improves overall survival. However, liver surgery continues to be associated with significant morbidity and mortality. A combination of careful patient selection, meticulous operative technique and specialized perioperative care is required to achieve low perioperative mortality (less than 5%) even in cases of hepatocellular carcinoma resections in cirrhotic patients. Postoperative hepatic failure (POLF) is one of the most serious complications following liver surgery with a mortality that can exceed 70%. The aim of this case scenario is to highlight the perioperative management of POLF after liver resection for hepatocellular carcinoma in patient with cirrhosis.

متن کاملUse of the liver maximum function capacity test (LiMAx) for the management of liver resection in cirrhosis – A case of hypopharyngeal cancer liver metastasis
INTRODUCTION The presence of liver cirrhosis goes along with a higher chance for the need of liver resection. As established laboratory parameters often underestimate the degree of cirrhosis this is associated with an increased risk for postoperative liver failure due to the preoperatively impaired liver function. Known liver function tests are unlikely to be performed in daily use because of h...
متن کاملTreatment of cirrhotic rats with epidermal growth factor and insulin accelerates liver DNA synthesis after partial hepatectomy.
Prevention of postoperative hepatic failure is important after hepatic resection. In patients with cirrhosis, impaired liver function and regenerative capacity after major hepatic resection are associated with increased morbidity and mortality. In this study, a combination of epidermal growth factor (EGF) and insulin were used as hepatotrophic factors in an attempt to stimulate DNA synthesis af...
متن کاملLiver surgery in the presence of cirrhosis or steatosis: Is morbidity increased?
BACKGROUND DATA The prevalence of steatosis and hepatitis-related liver cirrhosis is dramatically increasing together worldwide. Cirrhosis and, more recently, steatosis are recognized as a clinically important feature that influences patient morbidity and mortality after hepatic resection when compared with patients with healthy liver. OBJECTIVE To review present knowledge regarding how the p...
متن کاملReappraisal of Plasmapheresis as a Supportive Measure in a Patient with Hepatic Failure after Major Hepatectomy
Major resection of cirrhotic livers can result in hepatic failure, but no supportive treatment has been found to be generally effective. We successfully treated a 63-year-old woman with post-hepatectomy liver failure with plasmapheresis. Following right hepatectomy, the initial postoperative recovery of liver function was favorable, except for ascites. One month later, however, the amount of dr...
