Endobronchial ultrasound guided transbronchial needle aspiration of mediastinal lymph nodes for lung cancer staging: a projected cost analysis
نویسنده
چکیده
Endobronchial ultrasound guided-transbronchial needle aspiration (EBUS-TBNA) of mediastinal lymph nodes provides a safe alternative to mediastinoscopy for staging patients with lung cancer. Performing TBNA under real time ultrasound visualisation has been shown to significantly improve the yield of TBNA compared with conventional TBNAperformed without visualisation. In addition to the clinical benefits of this procedure, EBUSTBNA is likely to offer ongoing cost savings by avoiding the need for mediastinoscopy procedures and positron emission topography (PET) scans in some patients. At Leeds Teaching Hospitals NHS Trust (LTHT), conventional TBNA is used to assess large subcarinal nodes, but the Trust does not currently offer an EBUS-TBNA service. By reviewing mediastinoscopies performed as staging procedures for lung cancer in LTHT during 2006, we estimated the financial implications of establishing an EBUS-TBNA service.

متن کاملEndobronchial ultrasound–guided transbronchial needle aspiration in the staging of lung cancer patients
OBJECTIVE Mediastinoscopy as diagnostic procedure for evaluation of mediastinum in patients with non-small-cell lung cancer has long been considered the reference standard. However, less invasive method has occurred. Endobronchial ultrasound-guided transbronchial needle aspiration came into widespread use and has resulted in controversy as to whether it is a good replacement for mediastinoscopy...
متن کاملTransbronchial and transoesophageal (ultrasound-guided) needle aspirations for the analysis of mediastinal lesions.
A tissue diagnosis of mediastinal nodes is frequently needed for accurate lung cancer staging as well as the assessment of mediastinal masses. Transbronchial needle aspiration (TBNA) is a safe procedure that is performed during routine bronchoscopy. Provided mediastinal metastases are confirmed, TBNA has a high impact on patient management. Unfortunately, TBNA remains underused in current daily...
متن کاملClinical Implication of Microscopic Anthracotic Pigment in Mediastinal Staging of Non-Small Cell Lung Cancer by Endobronchial Ultrasound-Guided Transbronchial Needle Aspiration
Microscopic anthracotic pigment (MAP) is frequently observed in endobronchial ultrasound-guided transbronchial needle aspiration (EBUS-TBNA) specimen in non-small cell lung cancer, but its clinical interpretation is not well-known. The aim of this study was to evaluate the clinical implication of MAP in mediastinal staging of non-small cell lung cancer. From May 2010 to July 2011, consecutive p...

OBJECTIVE Endobronchial ultrasound-guided transbronchial needle aspiration is of diagnostic value in hilar/mediastinal (N1/N2) lymph node staging. We assessed the utility of endobronchial ultrasound-guided transbronchial needle aspiration in lung cancer patients with N1/N2 lymph nodes detected on (18)F-fluorodeoxyglucose positron emission tomography/computed tomography. METHODS Fifty lung can...
