Diagnosis of a displacement of Guglielmi detachable coils by transcranial color-coded sonography.

چکیده
n recent years, intracranial aneurysms that cause subarachnoid hemorrhage have been treated by an endovascular approach. The use of endovascular Guglielmi detachable coils (GDCs) has improved the radiologic and neurologic outcome of these patients, reaching results similar to those of craniotomy.1 This technique implies certain risks that are decreasing because of the development of new delivery systems and softer GDCs and the increasing experience of neuroradiologists.2 Displacement of coils is not an uncommon complication after endovascular embolization of cerebral aneurysms.3 Transcranial color-coded sonography (TCCS) is a noninvasive reliable bedside technique that allows an anatomic and hemodynamic follow up after embolization of aneurysms.4,5 We report the case of a patient in which a displacement of the coil occurred after GDC treatment of a cerebral aneurysm. A segment of the coil invaded the cerebral vessel, causing a partial blockage of blood flow. This complication was correctly diagnosed by TCCS, permitting a posterior endovascular solution.

Use of color power transcranial Doppler sonography to monitor aneurysmal coiling.
We describe the use of a recently developed technique in the field of color Doppler sonography, called power Doppler or color Doppler energy, that produces better images of the intracranial arteries than those obtained by conventional color Doppler techniques. Color Doppler energy makes it possible to identify aneurysms and their relationship to the parent artery, thus allowing one to observe h...
متن کاملTranscranial color-coded Doppler sonography of intracranial aneurysms before and after endovascular occlusion with Guglielmi detachable coils.
BACKGROUND AND PURPOSE Our purpose was to evaluate the ability of transcranial color-coded Doppler sonography (TCCD) to 1) identify Guglielmi detachable coils (GDCs) within intracranial aneurysms, 2) show endovascular aneurysmal occlusion and patency of parent and branch arteries, 3) determine the flow velocities within parent arteries and major branches before and after treatment, and 4) asses...
متن کاملExtracranial aneurysm and arteriovenous fistula: embolization with the Guglielmi detachable coil.
PURPOSE To evaluate the feasibility and safety of Guglielmi detachable coils for endovascular treatment of extracranial aneurysm and arteriovenous fistula. MATERIALS AND METHODS Embolization with Guglielmi detachable coils delivered through Tracker-18 microcatheters was performed in 16 patients. This patient group had eight renal artery aneurysms and 11 arteriovenous fistulas (three cases of ...
متن کاملIntracranial aneurysms treated with endovascular coils: detection of recurrences using unenhanced and contrast-enhanced transcranial color-coded duplex sonography.
BACKGROUND AND PURPOSE Because neck recurrence after endovascular treatment of intracranial aneurysms (IAs) is not uncommon, surveillance to assess long-term stability of occlusion is clearly important. This study evaluated unenhanced and contrast-enhanced transcranial color-coded duplex sonography (TCCS) in detecting refilling of IAs treated with detachable coils. METHODS Patients with coile...
متن کاملTopic review: surface modifications enhancing biological activity of guglielmi detachable coils in treating intracranial aneurysms.
BACKGROUND Endovascular therapy with Guglielmi detachable coils is an accepted treatment option for patients with intracranial aneurysms. However, an emerging technology in the realm of endovascular tools is the use of traditional Guglielmi detachable coils with biologically active substances complexed to the coil surface to enhance aneurysm occlusion. METHODS We review the literature and cur...
