Sporadic Balkan Endemic Nephropathy (BEN) beyond the known regions of BEN

چکیده
There is an very old opinion that we diagnose only most prominent BEN cases and miss latent and mild ones, that unrecognized BEN is possible in endemic villages and the number of such cases, their characteristics, and origin are unknown. From 1955 to 1998 year we evaluated 1235 patients with histologicaly proven upper urothelial tumors, who underwent surgery. The diagnosis of sporadic BEN, was made after exclusion of other known nephropaties. In the analyzed region, BEN was present in 170 of 808 settlements, and in 104 of them, 534 cases of UUT were reported. In 66 possible BEN settlements, there were no UUT cases. In the next 457 of 808 settlements (56.6%) neither BEN, or UUT were registered. Sporadic BEN is an atypical, mild clinical variety of BEN, with simultaneous presence of UUT. Both sexes were equally affected, with the majority of patients in sixth and seventh decade of. The presence of sporadic BEN cases gives a possibility to speculate that the existence of BEN without familiar agreggations is possible elsevere, not only in Balkans.

Environment, Medical Geology and the Etiology of Balkan Endemic Nephropathy
Background Balkan endemic nephropathy (BEN) is an irreversible, chronic, tubulo-interstitial nephropathy of unknown origin, geographically confined to several rural regions of the Balkan Peninsula. The first "official" observation of a disease resembling BEN was made almost six decades ago in regions comprising the former Yugoslavia (Danilovic et al., 1957). Shortly thereafter, similar descript...
متن کاملWhat do we know about the Balkan endemic nephropathy and the uroepithelial tumors?
Balkan endemic nephropathy (BEN), a familial chronic tubulo interstitial disease with a slow progression to terminal renal failure, affects people living in the alluvial plains along the tributaries of the Danube River. One of its most peculiar characteristics is a strong association with upper urothelial cancer. An increased incidence of upper urinary tract (UUT) transitional cell cancer (TCC)...
متن کاملUpper Urothelial Carcinomas Associated with Balkan Endemic Nephropathy and Their Similarities with Upper Urothelial Carcinomas in Analgesic Nephropathy
The incidence of upper urothelial carcinomas (UUC) is even more than hundred times higher in the foci of Balkan endemic nephropathy (BEN), by investigations of our team, and others. Every tenth patient suffers from both diseases. Etiology is unknown. It would seem that BEN is a tubulointerstitial nephritis associated with urothelial malignancy. We found that apoptosis plays an important role in...
متن کاملCurrent Status and Research Beyond Year 2010 in Balkan Endemic Nephropathy
In the first issue of BANTAO J, in 2003, I have presented the current research in Balkan endemic nephropathy (BEN) and associated upper urothelial tumors (UUT). As I stated in that paper, etiology remains the main problem for research in BEN [1]. Research of BEN in the nineties was hampered by the turmoil in the Balkans, especially in the former Yugoslavia. The Program Committee of the Interaca...

Balkan endemic nephropathy (BEN) was described in 1957, and this review gives an account of research forty years later. The etiology remains the main unanswered problem in BEN despite broad investigations conducted into the possible role of genetic factors, environmental agents and immune mechanisms. The evidence accumulated so far indicates that BEN is an environmentally-induced disease. A cont...
