Elimination of HIV in South Africa through expanded access to antiretroviral therapy: Cautions, caveats and the importance of parsimony

چکیده
In a recent article Hontelez et al. investigate the prospects for elimination of HIV in South Africa through expanded access to antiretroviral therapy (ART) using STDSIM, a micro-simulation model. One of the first published models to suggest that expanded access to ART could lead to the elimination of HIV, referred to by the authors as ‘the Granich Model’, was developed and implemented by the present author. The notion that expanded access to ART could lead to the end of the AIDS epidemic gave rise to considerable interest and debate and remains contentious. In considering this notion Hontelez et al. start by stripping down STDSIM to a simple model that is equivalent to the model developed by the present author but is a stochastic event driven model. Hontelez et al. then reintroduce levels of complexity to explore ways in which the model structure affects the results. In contrast to our earlier conclusions Hontelez et al. conclude that universal voluntary counselling and testing with immediate ART at 90% coverage should result in the elimination of HIV but would take three times longer than predicted by the model developed by the present author. Hontelez et al. suggest that the current scale-up of ART at CD4 cell counts less than 350 cells/μL will lead to elimination of HIV in 30 years. I disagree with both claims and believe that their more complex models rely on unwarranted and unsubstantiated assumptions.

Challenges and Proposed Actions for HIV/AIDS Control among Children in Iran
It is estimated globally 3.2 million children were living with HIV in 2014, that mostly in sub-Saharan Africa. However, the majority of them infected to HIV from their mothers during pregnancy, during delivery or breastfeeding (1). Antiretroviral therapy (ART) use during and after pregnancy is essential for prevention of mother-to-child transmission (PMTCT) of HIV (2). In the absence of any in...
متن کاملElimination of HIV in South Africa through Expanded Access to Antiretroviral Therapy: A Model Comparison Study
BACKGROUND Expanded access to antiretroviral therapy (ART) using universal test and treat (UTT) has been suggested as a strategy to eliminate HIV in South Africa within 7 y based on an influential mathematical modeling study. However, the underlying deterministic model was criticized widely, and other modeling studies did not always confirm the study's finding. The objective of our study is to ...

Objective(s): Broad range of metabolic changes associated with highly active antiretroviral therapy (HAART) has been reported over decades including reproductive perturbations. The current study aimed at investigating the role of Hypoxis hemerocallidea (Hyp) in the seminal and morphometric alterations in the testes of streptozotocin-nicotinamide-induced diabetic rats under HAART.Materials and M...

Objective(s): Diabetic nephropathy (DN) is an important primary cause of end-stage kidney disease. This study explores the mechanisms of the reno-protective effects of Momordica charantia (M. charantia) in diabetic rats following treatment with highly active antiretroviral therapy (HAART) regimen triplavar. متن کاملA study of barriers to adherence of antiretroviral treatment in prisoners living with HIV in Tehran, Iran
Background: People with chronic illnesses, such as HIV infection, face many barriers in the way of adherence to the treatment. In this study, the researchers attempted to investigate the barriers and facilitators of adherence to antiretroviral therapy (ART) among patients with HIV. Methods: Focus group discussion (FGD) was conducted with the primary objectives of investigating the barriers to ...
