Intrapleural haemorrhage in artificial pneumothorax.
نویسنده
چکیده
Intrapleural haemorrhage is a serious complication of artificial pneumothorax (A.P.), sometimes endangering life and often leading to obliterative pleurisy and loss of the A.P. It is the purpose of this paper to discuss its causes, incidence, pathology, treatment, and effect on the A.P., on the basis of a study of the records of 37 cases in which intrapleural haemorrhage of not less than one pint (600 ml.) occurred after refilling the A.P. or after thoracoscopy at the Brompton Hospital between 1939 and 1949.

In 1882, Forlanini12 of Milan proposed the induction of artificial pneumothorax in patients suffering from pulmonary tuberculosis, and in 1894’s he published his experience with the first patients to be submitted to this form of therapy. In 1885 Cayley had reported the case of a tuberculous patient in whom he had induced pneuniothorax by open incision of the chest wall to control hemoptysis, an...

shortened and the patient became disappointed. For this and other reasons it is probably best not to offer artificial pneumothorax as a substitute for bed rest but as an adjunct; to stress recovery rather than mere duration of time. Among other advantages are the following: As Compared With Bed Rest Alone—In the exudative case, bed rest should be instituted until localization occurs, when pneum...

more diseased part of the lung which is in greatest need of collapse. In the earlier years of artificial pneumothorax treatment attempts were made to sever these adhesions by thoracotomy (by the open method). This was attended with many com, plications and the procedure is not now considered justifiable. In 1910 Jacobeus introduced the present method of intrapleural pneumolysis and invented the...
متن کاملLife threatening tension pneumothorax during cardiac surgery. A case report
Tension pneumothorax is a life threatening condition that occurs when the intrapleural pressure exceeds atmospheric pressure. It requires prompt diagnosis and immediate treatment. Tension pneumothorax developing postoperatively after cardiac surgery is not uncommon but occurrence in the operating room during cardiac surgery is rare. We report a case of tension pneumothorax intraoperatively duri...
متن کاملDISEASES OF THE CHEST Closed Intrapleural Pneumonolysis
In the early stages of his pioneering work with artificial pneumothorax, Forlanini realised that effective collapse of tuberculous lungs was frequently prevented by the presence of adhesions between the visceral and parietal pleurae. He stated that in time the adhesions would often stretch sufficiently to allow a satisfactory collapse, but it was soon found that in many cases cavities remained ...
