چکیده
drops from neurologic disease, pilocarpine, a parasympathomimetic agent, is safe and reliable. It causes ·constriction of the iris sphincter unless atropine or an·other postsynaptic blocker, such as tropicamide, the substance involved in this case, has been used. Physostigmine salicylate (Antilirium) was not used, · but it is effective in treating anticholinergic intoxica.' tion when administered intramuscularly (l mg repeated in 15-20 minutes if necessary). In Ms. A's case, the anticholinergic effects of chlorpromazine, amitriptyline, scopolamine (Sominex) (0), and tropicamide were probably additive. Although her disorientation cleared rapidly, Ms. A maintained that her hallucinations persisted during her 2-week hospitalization. However, it is difficult to know : whether this was actually the case. Given the previous diagnosis of Munch~lUsen syndrome and her unusual willingness to assume the psychiatric patient role as well as the medical-surgical patient role, it is not unlikely that she was attempting to prolong her hospitalization.

Neuroleptic-induced supersensitivity psychosis: clinical and pharmacologic characteristics.
Tardive dyskinesia is thought to result from neostriatal dopaminergic receptor supersensitivity induced by chronic treatment with neuroleptics. The authors suggest that dopaminergic supersensitivity also occurs in the mesolimbic region after chronic neuroleptic exposure, resulting in the development of a supersensitivity psychosis. Neuroleptic-induced supersensitivity psychosis is illustrated b...

The author distinguishes between the putative neuroleptic-induced disorders of tardive dysmentia and supersensitivity psychosis. The distinction is made with regard to clinical features, response to pharmacological intervention, and proposed etiologies. It is suggested that tardive dysmentia is due not to dopaminergic supersensitivity but to some other neurotoxic phenomenon associated with the ...
متن کاملThe therapeutic latency of neuroleptic drugs and nonspecific postjunctional supersensitivity.
It is suggested that the antidopaminergic effects of neuroleptics are not directly responsible for the antipsychotic effect but, rather, that the antipsychotic effect is related to secondary changes in the efficacy of transmission at corticostriatal excitatory synapses. Arguments are presented in support of the following: (1) acute dopaminergic antagonism produces a relatively nonspecific sedat...

Although the time-course of development of dopaminergic supersensitivity has received some attention, there is little or no information on the rate of development of dopamine/neuroleptic receptors in the first days of neuroleptic treatment. For example, Christensen et al. ...
متن کاملPartial agonists in schizophrenia
The editorial by Bolonna & Kerwin (2005) is both timely and important. The authors succinctly present the case for the use of partial agonists of dopamine receptors, concluding that ‘the reviewed evidence suggests a promising future for dopamine receptor partial agonists’. While this is arguably true on the basis of the evidence presented, theoretical and empirical concerns regarding the use of...
