Autopsy diagnosis of decompression illness in rats by quantifying pulmonary emphysema

چکیده
Objectives. In medico-legal autopsy related to decompression illness (DCI) in divers, it is important to differentiate between postmortem decompression and antemortem decompression; however, definitive criteria for autopsy diagnosis of DCI remain to be established. We aimed to clarify autopsy findings that would aid the diagnosis of DCI. Methods. A total of 27 Wistar and 33 Zucker fatty male rats were categorized into 3 groups: antemortem decompression group (group AD), postmortem decompression group (group PD), and controls. The AD and PD rats were subjected to antemortem compression (both groups) followed by antemortem and postmortem decompression, respectively, in a hyperbaric chamber. Control rats were kept at atmospheric pressure. Intravascular bubbles and histopathological findings in the 3 groups were compared, and pulmonary emphysema was quantified using ImageJ software. Results. Intravascular bubbles were observed in both AD and PD groups. In group AD, prolonged hyperbaric exposure increased mortality, intravascular bubbles, and pulmonary emphysema, with Zucker fatty rats showing more marked changes than Wistar rats. Further, intravascular bubbles and pulmonary emphysema in the dead rats were quantitatively greater than those in the rats that survived decompression. Moreover, in Zucker fatty rats, pulmonary emphysema was quantitatively significantly greater in group PD than that in the dead rats of group AD. Conclusion. Our results indicate that the quantitative evaluation of pulmonary emphysema can potentially distinguish postmortem decompression from antemortem decompression. This technique may be useful in autopsy diagnosis of divingrelated deaths, including DCI.

متن کاملSubcutaneous emphysema, pneumomediastinum, and potentially life-threatening tension pneumothorax. Pulmonary complications from arthroscopic shoulder decompression.
Subcutaneous emphysema, pneumomediastinum, and tension pneumothorax are previously unreported complications of shoulder arthroscopy with subacromial decompression. Three patients developed extensive subcutaneous emphysema, pneumomediastinum, and bilateral tension pneumothorax during or immediately after shoulder arthroscopy with subacromial decompression. The procedure was terminated and approp...

Barotraumas and decompression sickness are the two most well-known complications of diving. First presented case was 32 year-old male with recreational diver, who was found floating prone position on the bottom of sea in a depth of 33 m. He had been carried to the surface in a controlled ascent. Second case was a 39 year-old male experienced dive instructor in a diving school, after following a...
متن کاملPneumomediastinum and subcutaneous emphysema in status asthmaticus, requiring surgical decompression.
Leakage of air into the interstitial tissues of the lungs of sufficient degree to produce pneumomediastinum and subcutaneous emphysema is an uncommon and potentially fatal complication of bronchial asthma in childhood. Another possibly fatal complication of similar causation is tension pneumothorax. It is possible that interstitial emphysema occurs more frequently in severe asthma and allied st...
متن کاملRisk Factors for Pulmonary Barotrauma in
Study objectives: Pulmonary barotrauma (PBT) of ascent is a feared complication in compressed air diving. Although certain respiratory conditions are thought to increase the risk of suffering PBT and thus should preclude diving, in most cases of PBT, risk factors are described as not being present. The purpose of our study was to evaluate factors that possibly cause PBT. Design: We analyzed 15 ...
