چکیده

OBSTRUCTION to left ventricular outflow has been considered a characteristic and functionally important feature of "hypertrophic subaortic stenosis." The presence of obstruction has been inferred from the finding of a pressure gradient within the left ventricle during catheterization of the left heart in certain patients with idiopathic left ventricular hypertrophy. Indeed, the disease was not recognized as a clinical entity until after the advent of left heart catheterization technics. Infrequent descriptions of massive hypertrophy with virtual obliteration of the left ventricular cavity in autopsy reports dating back to 1907,1 lend some support to the concept of muscular obstruction. A number of observations render the functional importance of obstruction less clear in "hypertrophic subaortic stenosis" than in discrete forms of aortic stenosis. 1. There is no correlation between the size of the left ventricular gradient ("severity of the obstruction") and the functional status of the patient.2 2. The pressure gradient is quite variable and often absent during the course of a left heart catheterization study or on repeated studies. 3. No obstruction is found at operation or at autopsy in some patients with large pressure gradients.

Comparison of ventricular emptying with and without a pressure gradient in patients with hypertrophic cardiomyopathy.

Thirty three patients with hypertrophic cardiomyopathy were studied to determine whether the presence of an intraventricular pressure gradient impaired left ventricular emptying. Patients with resting gradients had a higher mean left ventricular ejection fraction (92 (6.4)%) than patients without a resting or inducible pressure gradient (75.5 (9)%). The rate and degree of emptying increased whe...

Fluid dynamics of aortic stenosis: subvalvular gradients without subvalvular obstruction.

Analysis of a tapering, pulsatile flow field predicts that substantial subvalvular pressure gradients exist in patients with valvular aortic stenosis (AS) without invoking a second anatomic site of obstruction. Using a catheter with two laterally mounted micromanometers, we examined the left ventricle in 11 patients with AS, mean age 64 +/- 11 years (+/- SD); the mean valve area was 1.0 +/- 0.3...

Non-invasive estimation of pulmonary outflow tract obstruction: A comparative study of cardiovascular phase contrast magnetic resonance and Doppler echocardiography versus cardiac catheterization

AIM To compare estimated pressure gradients from routine follow-up cardiovascular phase-contrast magnetic resonance (PC-MR) with those from Doppler echocardiography and invasive catheterization in patients with congenital heart disease (CHD) and pulmonary outflow tract obstruction. METHODS In 75 patients with pulmonary outflow tract obstruction maximal and mean PC-MR gradients were compared t...

Obstruction is unimportant in the pathophysiology of hypertrophic cardiomyopathy

There has been a longstanding controversy about the significance of intracavitary pressure gradients in hypertrophic cardiomyopathy (HCM). It has been generally assumed that the gradient is the result of an 'obstruction' that impedes left ventricular outflow and which can be relieved by operative intervention. In the first decade after the discovery of HCM (1957-66), the site of 'obstruction' w...

Significance of left ventricular inflow gradients in patients with ventricular septal defect.

Hemodynamic data of 167 patients of isolated ventricular septal defect (VSD) was retrospectively analyzed for the presence of left ventricular inflow gradients. End diastolic gradients of > 5 mm Hg between the pulmonary artery wedge pressure and the left ventricular end diastolic pressure were recorded in 40 of these patients. In three of these cases, left atrium was also entered and identical ...
