Are there really biomarkers of vulnerable plaque?

چکیده

The rationale for the search of biomarkers of vulnerable plaques seems sound. Plaque rupture is a key step in the initiation of an acute coronary syndrome (ACS). The atherosclerotic plaque contains large numbers of inflammatory cells that may release hydrolytic enzymes and cytokines, thereby destabilizing the plaque (1 ). Therefore, many inflammatory biomarkers and enzymes have been suggested as potential biomarkers of vulnerable plaques; however, considerable confusion exists in the literature regarding what is meant by “a biomarker of plaque vulnerability,” and other closely related (but not identical) concepts have emerged as alternatives. These concepts are described briefly below.

The vulnerable patient: refocusing on the plaque?

The term 'vulnerable plaque' is used to refer to the lesions that are prone to rupture and may cause life-threatening events like acute coronary syndrome or stroke. The study of the vulnerable plaque phenotype and its detection has attracted increasing interest over the past decades. During this time, there have been some remarkable transitions in the paradigm on methods to identify patients at...

Relationship between multiple plasma biomarkers and vulnerable plaque determined by virtual histology intravascular ultrasound.

BACKGROUND The relationship between plasma biomarkers and vulnerable plaque is not well understood. METHODS AND RESULTS The 188 patients who underwent 3-vessel virtual histology (VH) intravascular ultrasound (IVUS) with peripheral blood sampling were enrolled. Plasma levels of matrix metalloproteinase 2 and 9 (MMP-2, -9), tissue inhibitor of metalloproteinase-1, adiponectin, and macrophage mi...

Atherosclerotic Plaque Biomarkers: Beyond the Horizon of the Vulnerable Plaque

Cardiovascular disease (CVD) is the number one cause of death globally, and the majority of CVD is caused by atherosclerosis. Atherosclerosis is a systemic inflammatory disease that leads to myocardial infarction, stroke and lower limb ischemia. Pathological studies have given insight to development of atherosclerosis and the importance of local plaque vulnerability, leading to thrombus formati...

Biobanks and the search for predictive biomarkers of local and systemic outcome in atherosclerotic disease.

Multiple risk factors have been associated with progression of atherosclerosis. To identify the individual patient who is at risk for disruption of a vulnerable plaque, leading to a cardiovascular event, remains a major challenge. Current screening methods, based on traditional risk factors, do not allow risk stratification on an individual level. The discovery of new biomarkers would aid in id...

Pregnancy-associated plasma protein-A and the vulnerable plaque.

For more than a decade, pregnancy-associated plasma protein-A (PAPP-A) has been examined for its relation to acute coronary syndrome (ACS) and the vulnerable plaque. This review summarizes the current knowledge of plasma PAPP-A in relation to nonpregnant individuals focusing on patients with ACS, discusses its use as a possible biomarker for diagnosis and prognosis in ACS, briefly describes the...
