چکیده

Method Between 2010 and 2014, 182 patients underwent of extended myectomy procedures. 70 patient met inclusion criteria: were randomly assigned to receive MSA intervention in addition to septal myectomy (MSI group; n = 36) or undergo septal myectomy only (without MSI group; n = 34). A primary HOCM was the main indication for surgery according to 2011 ACCF/AHA guidelines. Mean age was 52.8 ± 14.2 years (range 22 to 74 years). Mean peak gradient was 90.7 ± 24.2 mm Hg. Mean thickness of interventricular septum was 26.1 ± 4.3 mm. SAM syndrome observed in all patients. MR: moderate 42 (60%) pts, severe 28 (40%) pts.

Sustained improvement after combined anterior mitral valve leaflet retention plasty and septal myectomy in preventing systolic anterior motion in hypertrophic obstructive cardiomyopathy in children.

OBJECTIVE Anatomic alterations of the mitral valve such as increased mitral leaflet area, length and laxity, and anterior displacement of the papillary muscles in hypertrophic obstructive cardiomyopathy predispose patients to residual systolic anterior motion and persistence of outflow obstruction and mitral regurgitation after septal myectomy. We investigate the long-term results of combined a...

Muscular (hypertrophic) subaortic stenosis (hypertrophic obstructive cardiomyopathy): the evidence for true obstruction to left ventricular outflow.

The clinical and haemodynamic significance of the subaortic pressure gradient in patients with muscular (hypertrophic) subaortic stenosis (hypertrophic obstructive cardiomyopathy) has long been debated. In this report we summarize the evidence which indicates that true obstruction to left ventricular outflow exists in these patients. Rapid left ventricular ejection, through an outflow tract nar...

It's not just the mitral valve - abnormal motion of the whole aorto-mitral apparatus occurs in both overt and subclinical hypertrophic cardiomyopathy

Background Mitral valve abnormalities are an important cause of patient morbidity in hypertrophic cardiomyopathy (HCM) contributing to left ventricular outflow tract obstruction (LVOTO). Anterior mitral valve leaflet (AMVL) elongation predisposes to LVOTO. CMR detects this in overt HCM and in sarcomere gene mutation carriers without left ventricular hypertrophy (G+LVH-). However, the geometry o...

Surgical treatment of hypertrophic obstructive cardiomyopathy.

The complexity of the anatomical substrate that produces obstruction in hypertrophic obstructive cardiomyopathy (HOCM) is reflected by the variety of different surgical strategies that have been reported during the past 30 years[1–12]. These include incision (myotomy)[1] and excision (myectomy)[2,3] of bulging myocardial tissue from the basal septum, in some series together with plication[9] or...
