MR-guided laser-induced thermotherapy (LITT) of liver metastases: Comparison of colorectal cancer mets versus breast cancer mets and other primary tumors regarding survival rates

چکیده
Methods and Materials: 278 consecutive patients 28 to 84 years of age (mean 59 y) with a total of 822 liver metastases were treated with MR-guided LITT under local anesthesia. 533 liver metastases of colorectal cancer (175 patients), 126 liver metastases of breast cancer (48 patients) and 163 metastases of a variety of other primary tumors (55 patients) were treated with a total of 2138 laser applications. 1000 laser applications were performed by the use of a conventional laser application set (SOMATEX, Berlin Germany). For 1138 laser applications an irrigated internally cooled power laser application system (SOMATEX, Berlin, Germany) was used. MR thermometry was performed using the T 1 -weighted thermometry method. A FLASH-2D sequence (TR/TE/flip angle = 102/8/70”) was used in all patients. Cumulative survival times were calculated with the KaplanMeier method.

MR-guided laser-induced interstitial thermotherapy (LITT) is a percutaneous, minimally invasive treatment modality for treating liver lesions/metastases, soft tissue tumours and musculoskeletal lesions. In this group, MR-guided LITT is currently performed under local anaesthesia on an out-patient basis with a specially designed saline-cooled laser application system. Nd:YAG laser (1064 nm wave ...

Purpose: To evaluate a treatment protocol with repeated transarterial chemoembolization (TACE) before laser-induced thermotherapy (LITT) in patients with unresectable liver metastases too large for LITT alone. Materials and Methods: A total of 162 patients with unresectable liver metastases (the largest lesion smaller than 80 mm in diameter, no more than 4 lesions) were treated with repeated TA...

The liver plays a central role in the human metabolism and so represents one of the organ systems most often affected, especially by tumorous diseases. The group of colorectal carcinomas metastatically almost exclusively attacks this organ, which, according to studies by Weiss et al., can be attributed to the venous drainage of the intestines through the portal vein [1,2]. A large number of pri...
