چکیده

Left bundle branch block (LBBB) is an electrophysiological abnormality that can have profound acute and chronic hemodynamic effects. Intermittent LBBB has been noted to occur with accelerations in heart rate, typically during exercise (known as rate-dependent LBBB). This has been associated with acute deteriorations in cardiac systolic function through the development of left ventricular (LV) dyssynchrony, and also episodic chest pain (“painful LBBB syndrome”). We present a case of an acute rate-dependent LBBB-associated cardiomyopathy with recurrent episodes of systolic heart failure and “flash” pulmonary edema that was successfully treated with cardiac resynchronization therapy (CRT).

The Clinical Significance of Bundle Branch Block Complicating Acute Myocardial Infarction

To provide an understanding of the clinical characteristics of patients with acute myocardial infarction (MI) and bundle branch block, experience from five centers was accumulated. Patients in whom bundle branch block first appeared after the onset of cardiogenic shock were excluded. In 432 patients, the most common types of block were left (38%) and right with left anterior fascicular block (3...

Transient dyspnea and left bundle branch block: a case of Munchausen Syndrome

This case report illustrates the electrocardiographic and two-dimensional (2D) echocardiographic findings of Flecainide toxicity in a young female with Munchausen syndrome, demonstrating the appearance of a septal flash, interventricular dyssynchrony and a complete left bundle branch block, all of which resolved following drug withdrawal. Case Report A 39 year-old hospital nurse presented to th...

The pathogenesis of atrioventricular block in coronary disease.

SUMMARY The hypothesis that the pathogenesis of acute and chronic atrioventricular (A-V) block in coronary disease is different is investigated. The conduction systems of two cases of acute A-V block and four cases of chronic A-V block associated with coronary disease were studied by serial section. This showed that acute blocks in coronary disease are related to ischemia. Chronic blocks, howev...

Transient disappearance of left bundle branch block pattern: an unusual ECG presentation of acute pulmonary embolism.

A 61 year old man developed acute pulmonary embolism while in hospital. His previous and admission electrocardiograms (ECGs) showed a typical left bundle branch block (LBBB) pattern. Immediately after the onset of acute pulmonary embolism, LBBB disappeared from his body surface ECG with sinus bradycardia, normalisation of QRS duration, prolonged QT interval, and marked T abnormalities to the ri...
