چکیده
Fig 1: Midsagittal section through brainstem showing prominent midbrain atrophy without pontine atrophy resembling a hummingbird (arrow showing the beak) and a penguin Fig 2: Axial MRI showing selective atrophy of the midbrain tegmentum, with relative preservation of the tectum and cerebral peduncles resembling Mickeymouse (Arrow) A fifty year old male was brought by his wife with insidious onset forgetfulness, slurring of speech, slowness of movements and frequent falls. There was no history of vomiting, seizures, limb weakness, diplopia, dysarthria, dysphagia, nasal regurgitation or facial asymmetry. Examination findings were remarkable for borderline impairment of memory (MMSE score= 27 / 30), slurred speech, conjugate vertical gaze palsy, slowness of horizontal saccades and bradykinesia. Glabellar tap was positive. His blink rate was 6-8/min. Cogwheel rigidity, tremors or autonomic features were not present in this patient.MRI Brain revealed flattening of superior profile of midbrain due to midbrain tegmental atrophy with widening of interpedencular cistern giving the impression of a hummingbird and Mickey mouse appearance of the brainstem (Figures 1 and 2).Based on the clinical and radiographic features , he was diagnosed as a case of Progressive Supranuclear Palsy ( Steele-Richardson-Olszewski syndrome). Progressive supranuclear palsy (PSP) is a form of Parkinsonism which is a neurodegenerative disorder. Along with diseases like multisystem atrophy, corticobasal degeneration, dementia with Lewy bodies and frontotemporal dementia, it constitutes the Parkinsonplus syndromes. The ensuing Table 1 compares this disorder with idiopathic Parkinson disease

Jerome J. Graber, MD, MPH Robert Staudinger, MD, FAAN An 82-year-old man presented to us with a 2-year history of progressive difficulty walking. On examination, he had bradykinesia, bradyphrenia, axial rigidity without appendicular rigidity, cogwheeling, or tremor, and restriction of upward and downward gaze. Imaging revealed prominent midbrain atrophy without pontine atrophy, referred to as t...
متن کاملThe Hummingbird sign: a diagnostic clue for Steele-Richardson-Olszweski syndrome.
DESCRIPTION Progressive supranuclear palsy (PSP), previously known as Steele-Richardson-Olszweski syndrome, is an atypical parkinsonian syndrome with a prevalence of ∼5/100 000. It is an important differential diagnosis of more common idiopathic Parkinson’s disease (iPD), where clinical differentiation is not straightforward and characteristic neuroimaging often yields a diagnostic clue. We des...
متن کاملHummingbird sign in progressive supranuclear palsy disease
Progressive supranuclear palsy (PSP) is characterized by slowness, rigidity, bradykinesia, repeated falls, downgaze limitation and dementia. Midbrain atrophy on magnetic resonance imaging is highly suggestive of PSP and is described as "hummingbird sign". This sign is very helpful in differentiating PSP patients from those with Parkinson's disease. We hereby report a 72-year-old female case of P...

– Diagnosis in neuroimaging involves the recognition of specific patterns indicative of particular diseases. Pareidolia, the misperception of vague or obscure stimuli being perceived as something clear and distinct, is somewhat beneficial for the physician in the pursuit of diagnostic strategies. Animals may be pareidolically recognized in neuroimages according to the presence of specific disea...
متن کامل‘Hummingbird’ Sign in a Patient with Guam Parkinsonism-Dementia Complex
We present a case of a 71-year-old male Chamorro patient from Guam who presented with progressive supranuclear palsy (PSP)-Richardson's syndrome. Considering his strong family history of parkinsonism and a PSP phenotype, he was clinically diagnosed with Guam parkinsonism-dementia complex (PDC). Magnetic resonance imaging (MRI) of the brain revealed prominent midbrain atrophy with preserved pont...
