High-Resolution Mapping of Ventricular Scar

ثبت نشده
چکیده

The pathophysiology of infarct-related reentrant ventricular tachycardia (VT) includes myocardial cell death and ventricular remodeling, resulting in inhomogeneous scarring with variable degrees and configurations of surviving myocardial bundles within the areas of fibrosis. This provides the necessary electrophysiological substrate for formation of reentrant VTs that are predominantly located in the subendocardium.

Correlation of Scar in Cardiac MRI and High‐Resolution Contact Mapping of Left Ventricle in a Chronic Infarct Model

BACKGROUND Endocardial mapping for scars and abnormal electrograms forms the most essential component of ventricular tachycardia ablation. The utility of ultra-high resolution mapping of ventricular scar was assessed using a multielectrode contact mapping system in a chronic canine infarct model. METHODS Chronic infarcts were created in five anesthetized dogs by ligating the left anterior des...

Catheter ablation of hemodynamically unstable ventricular tachycardia in ischemic cardiomyopathy using high‐resolution mapping

Catheter ablation is a recommended therapy option for ventricular tachycardia (VT). The antegrade transseptal approach for targeting VT with left ventricular origin is feasible with the high-resolution basket catheter. High-resolution mapping offers the potential to quickly acquire detailed voltage and activation maps. This may help to identify the crucial VT-substrate even in patients with hug...

Feasibility of real time integration of high-resolution scar images with invasive electrograms in electro-anatomical mapping system in patients undergoing ventricular tachycardia ablation

Background Ventricular tachycardia (VT) ablation is generally guided by invasive mapping of the left ventricle (LV) using electro-anatomical voltage mapping (EAM) to identify the VT substrate [1]. Late gadolinium enhancement (LGE) MRI allows excellent visualization of the scar. Heterogeneous area in LGE images has been shown to correlate with the VT substrate in animal models of VT. Retrospecti...

Imaging surrogate of the ventricular arrhythmia substrate in a swine model of ventricular tachycardia: high resolution LGE vs. high resolution electro-anatomical mapping

Background Assessment of risk of sudden cardiac death (SCD) due to ventricular tachycardia and fibrillation (VT/VF) remains one of the most challenging areas in management of patients with chronic myocardial infarction. VT substrate consists of reentry circuits near or within a chronic scar. Invasive catheter mapping using electroanatomical system (EAM) is the clinical standard for identificati...
