We read with great interest the paper by Blazek et al. (1) in this issue of JACC: Cardiovascular Interventions. This is an mportant contribution to the debate on which treatment odality is best for lesions that involve the left anterior escending artery (LAD). Although percutaneous coronary ntervention (PCI) offers a much less invasive treatment ption compared with coronary artery bypass graft surgery, he long-term patency rate of the left internal mammary rtery (LIMA) to LAD graft has rendered the LIMA–LAD s the gold standard for revascularization of the LAD (2). ts patency rate can reach as high as 95% to 98% at 10to 0-year follow-up.
