Teaching video NeuroImages: the breathing arm: respiratory brachial synkinesis.

چکیده
A 19-year-old man sustained a right upper trunk brachial plexopathy from a stab wound and became paralyzed in the right proximal arm. Twelve months later, he underwent microsurgical anastomosis of injured C5 and C6 nerve roots to the more distal C5 nerve root. At 14 months postoperatively, there were involuntary movements of his right biceps and deltoid muscles time-locked to inspiration (see video on the Neurology Web site at www.neurology.org; see figure). The diaphragm receives innervation from C3–C5 nerve roots via the phrenic nerve. During regeneration of motor neurons originally destined for the phrenic nerve, aberrant regrowth into the upper trunk of the brachial plexus resulted in reinnervation of the biceps brachii and deltoid (via musculocutaneous and axillary nerves), rather than the phrenic nerve. The result is involuntary contraction of the biceps and deltoid muscles time-locked to breathing or the “breathing arm” syndrome. This phenomenon has been observed after surgery or trauma, including obstetric brachial plexus injuries. Congenital variants also exist, including trigemino-abducens synkinesis.1-3

Teaching NeuroImages: unilateral upper limb muscular hyperplasia.
A 64-year-old woman had a painful left hand her entire life. Examination revealed hypertrophy of the left arm and hand with an increased muscle bulk of the first dorsal interosseous muscle and ulnar deviation of digits II to V (figures 1 and 2). Except for large motor unit potentials with a normal interference pattern, EMG was normal. Brain imaging was normal. Unilateral upper limb muscular hyp...
متن کاملTeaching Video NeuroImages: complex partial seizure evolving into a psychogenic nonepileptic seizure.
An 11-year-old, right-handed girl was admitted for video-EEG monitoring of nocturnal paroxysms consisting of eye opening, groans, right arm shaking, and altered consciousness (see video on the Neurology Web site at www.neurology.org). Ictal video-EEG suggested a left frontal seizure; subsequent behavioral disturbances including dysrhythmic right arm shaking and confusion continued without elect...
متن کاملTeaching Video NeuroImages: Movement of a paralyzed arm with yawning.
A 63-year-old man presented with aphasia and right arm paralysis (Medical Research Council [MRC] grade 0), sparing the leg (video 1 on the Neurology® Web site at Neurology.org). With yawning, his right arm consistently rose to his chest (video 2). Paralysis resumed after yawning. MRI confirmed an acute ischemic stroke in the left middle cerebral artery distribution (figure). This movement cease...
متن کاملCase Report: A Case Report on Variation in Bifurcation of Brachial Artery
Variations in arterial anatomy are less frequent, contrary to the venous system, and most of these variations affect visceral arteries. Variations in the brachial artery are the most frequently reported and so far a minimum of six different patterns have been described. The most common of these patterns is the superficial brachial artery, which lies superficially to the medi...
متن کاملTeaching NeuroImages: median nerve MRI changes over time in neuralgic amyotrophy.
A 40-year-old woman presented with 2 weeks of left arm pain followed by hand weakness and numbness. Electrodiagnostic testing revealed an isolated median sensorimotor axonopathy localizing proximal to the pronator teres. The working diagnosis of a forme fruste of neuralgic amyotrophy was made. MRI of the left arm was performed to exclude rare structural causes of proximal median neuropathy. The...
