New ECG criteria for high-risk Brugada syndrome.

چکیده
To identify high-risk patients with Brugada syndrome, the present study reviewed 60 standard 12-lead electrocardiograms from 60 patients collected by the Japanese Brugada syndrome registry. Under blinded conditions, the S wave of lead V(1) was measured from the tip of r to r', and the amplitude of the ST segment in lead V(2) was measured at 0.08 s from the J point. In patients with ventricular fibrillation (n=17), the S wave was significantly longer in V(1) (0.085+/-0.007 s vs 0.075+/-0.011 s, p=0.001), and ST segment elevation in V(2) was significantly greater (0.323+/-0.133 mV vs 0.236+/-0.129 mV, p=0.012) than in patients without fibrillation. An S wave width of 0.08 s or more in V(1) had a positive predictive value of 40.5% and negative predictive value of 100% for ventricular fibrillation, with 100% sensitivity. ST elevation of 0.18 mV or more in V(2) had a positive predictive value of 37.8% and a negative predictive value of 100% for ventricular fibrillation, with 100% sensitivity. Both an S wave width > or =0.08 s in V(1) and ST elevation > or =0.18 mV in V(2) were highly specific indicators of ventricular fibrillation and are proposed as new criteria for high-risk Brugada syndrome.

Approach to the asymptomatic patients with Brugada syndrome
Brugada syndrome is an arrhythmogenic disease characterized by an ECG pattern of coved-type ST segment elevation in the right precordial leads and an increased risk of sudden cardiac death (SCD) as a result of polymorphic ventricular tachyarrhythmia or ventricular fibrillation (VF). Data from large patient studies and a meta-analysis of previous reports have shown that patients with a history o...
متن کاملA case of Brugada Syndrome unmasked by a postoperative febrile state
Abstract Background: Brugada Syndrome (BS) is an inherited ion channelopathy characterized by an electrocardiographic (ECG) pattern of a coved type ST segment elevation in right precordial leads with or without right bundle branch block. Case Presentation: A 23-year old male presented with right lower quadrant abdominal pain. Further evaluation revealed a diagnosis of acute appendicitis. The ...

Since its introduction as a clinical entity in 1992, the Brugada syndrome has progressed from being a rare disease to one that is second only to automobile accidents as a cause of death among young adults in some countries. Electrocardiographically characterized by a distinct ST-segment elevation in the right precordial leads, the syndrome is associated with a high risk for sudden cardiac death...
متن کاملA tick-borne illness unmasking asymptomatic Brugada syndrome
Brugada syndrome is an important cause of sudden cardiac death and should be recognized in asymptomatic patients with characteristic electrocardiographic (ECG) findings. We report a case of a 59-year-old male who presented with fever and generalized malaise after a camping trip with confirmed tick exposure. Initial diagnostic work-up included an ECG, which showed incidental ST-segment elevation...
متن کاملBrugada Syndrome and Exercise Practice: Current Knowledge, Shortcomings and Open Questions.
Since its recognition as a clinical entity in 1992, the Brugada Syndrome (BrS), a hereditary disease characterized by a typical electrocardiogram (ECG) pattern potentially predisposing to sudden cardiac death (SCD), has attracted the attention of many physicians for its circadian pattern of ventricular arrhythmias (VA), mostly occurring at rest. Exercise may potentially worsen the ECG abnormali...
متن کاملNew electrocardiographic criteria to differentiate the Type-2 Brugada pattern from electrocardiogram of healthy athletes with r'-wave in leads V1/V2.
AIMS Diagnosis of Type-2 Brugada pattern remains challenging and it could be confused with other electrocardiogram (ECG) patterns presenting an r'-wave in leads V1-V2 like in healthy athletes. This could impact their ability to perform competitive sports. The aim of the study was to evaluate, as a proof of concept, the new ECG criteria to differentiate the Type-2 Brugada pattern from the ECG pa...
