Pre- and Intra-operative Planning and Simulation of Percutaneous Tumor Ablation

چکیده
We developed a software tool for pre-operative simulation and planning, and intra-operative guidance, of minimally invasive tumor ablation, including radiofrequency-, laserand cryo-therapy. This tool provides a preand intra-operative optimization of the treatment plan, in order to avoid dangerous probe trajectories, undertreatment of the tumor, and excessive ablation of healthy tissues. The simulation is performed within a virtual operating-room consisting in essence of the patient's segmented anatomy from preor intraoperatively acquired MR scans. Virtual probes can be placed into this scene and at the formation of ablated tissue at their tips can be simulated. To verify the simulated treatment plans, we introduced an objective quality measure which also enables a semi-automated optimal probe placement. To show the use and to underline the importance of our tool, we investigated a cryo-therapy case which did not succeed. We show that our software would have predicted the failure of the chosen treatment plan and how it could have increased the e cacy of the procedure.

Background: Intra-operative radiation therapy (IORT) is the transfer of a single large radiation dose to the tumor bed during surgery with the final goal of improving regional tumor control. This study aimed to investigate the safety, effectiveness and economic evaluation of intra-operative radiation therapy. متن کاملChallenges to Validate Multi-physics Model of Liver Tumor Radiofrequency Ablation from Pre-clinical Data
The planning and interventional guidance of liver tumor radiofrequency ablation (RFA) is difficult due to the cooling effect of large vessels and the large variability of tissue parameters. Subject-specific modeling of RFA is challenging as it requires the knowledge of model geometry and hemodynamics as well as the simulation of heat transfer and cell death mechanisms. In this paper, we propose...

Background: Local ablative treatments play an important role for patients who cannot be treated surgically. Radiofrequency ablation is a well-established alternative to surgical treatment of thyroid nodules, however it also has disadvantages. Microwave ablation (MWA) is a new minimally invasive treatment promising several improvements. The aim of this retrospective study was to evaluate the eff...

We introduce an automated and accurate system for registering pre-operative 3D MR and CT images with intraoperative 3D ultrasound images based on the vessels visible in both. The clinical goal is to guide the radio-frequency ablation (RFA) of liver lesions using percutaneous ultrasound even when the lesions are not directly visible using ultrasound. The lesions locations and desired RFA sites a...
