Effects of Direct-interrupting Surgery on Gastric Mucosal Congestion in Patients with Esophageal Varices

چکیده
The purpose of this study is to clarify the effect of direct-interrupting surgery on gastric mucosal congestion in patients with esophageal varices. Fifteen such patients underwent endoscopy in the preoperative, early postoperative and late postoperative periods. The index of local blood volume (lHb) and that of oxygen saturation (1802) were endoscopically measured using a tissue reflectance spectrophotometer at the antrum, the angle, and the corpus of the stomach. The congestion index (CI) was calculated by dividing IHb by 1802. At the antrum and the angle, none of the mean values of IHb, 1802, CI, or the incidence of mucosal redness changed significantly. At the corpus, the mean values of IHb and CI in the early postoperative period were significantly lower than those in the preoperative period (p < 0.01 and p<0.05, respectively), but those in the late postoperative period showed no significant differences from preoperative and early postoperative values. Also at the corpus, the mean value of 1802 and the incidence of mucosal redness showed no significant change in the early and late postoperative periods. These results suggest that gastric mucosal congestion of the corpus of the stomach in patients with esophageal varices can be relieved in the early postoperative period although it may recur in the late postoperative period.

SUMMARY Bleeding of esophageal varices have a high mortality rate, and medical management is usualy unsuccessful. Surgical methods such as vascular ligation or shunting are expensive. High risk procedures with high mortality. Endoscopic sclerotherapy was applied in 104 patients with acute bleeding of esophageal varices during a period of 5 years in a prospettive and retrospective study. Th...

BACKGROUND A significant number of patients with schistosomiasis develop the hepatosplenic form, with portal hypertension, in which bleeding caused by rupture of esophagogastric varices emerged as the leading cause of morbidity and mortality. AIM To investigate the effects of splenectomy and ligature of the left gastric vein on risk factors for bleeding of esophagogastric varices in patients ...

Background: Chronic pancreatitis and perivasculitis is the most common etiology of splenic vein thrombosis (SVT). Reported in up to 45% of patients with chronic pancreatitis, SVT may also be seen in patients with acute pancreatitis and pancreatic adenocarcinoma. It causes a localized portal hypertension called sinistral portal hypertension. Unlike those with generalized portal hypertension, pat...

Patients with Barrett's esophagus (BE) and cirrhosis who develop high-grade dysplasia (HGD) or adenocarcinoma in the setting of esophageal varices present a unique therapeutic dilemma. There is limited literature regarding the optimal management of varices prior to invasive procedures or surgery involving the distal esophagus. We present a case of variceal decompression with a transjugular intr...
