Teaching video neuroimages: hypothalamic hamartoma: an overlooked entity.

چکیده
Neeraj N. Baheti, DM Ajith Cherian, DM Ramshekhar Menon, DM Ashalatha Radhakrishnan, DM A 21-year-old man presented with brief spells of inappropriate laughter since 6 years of age. Three gelastic seizures were recorded during video-EEG monitoring (video on the Neurology Web site at www.neurology. org). MRI revealed a small hypothalamic hamartoma (figure 1). Gelastic seizures can also arise from temporal lobe and mesial frontal regions.1 Meticulous scrutiny of MRI is needed since epileptogenic hamartomas may be small and nonpedunculated, and therefore easily missed. Surgical removal of hamartoma is very effective in controlling seizures.2 Histopathology shows variable-sized nodular foci of neurons as well as areas of diffusely distributed neurons interspersed among glial cells without dysplastic neurons (figure 2).

Teaching neuroImages: short stature, imperforate anus, and polydactyly: When is a hypothalamic mass an incidentaloma?
A 4-year-old girl with history of polydactyly and imperforate anus surgeries presented with short stature. Brain MRI revealed a large hypothalamic hamartoma (figure). Pallister-Hall syndrome (PHS) was diagnosed. Biopsy was not indicated given stable and typical tumoral appearance. Annual evaluations, hormonal replacement, and medical genetics consult were recommended. Compared to sporadic hypot...

Antonio J. da Rocha, PhD Marcos Rosa Junior, MD Fernando Norio Arita, PhD Isolated hypothalamic hamartomas (HH) have a distinct clinical phenotype from that of PallisterHall syndrome (PHS), as HH consist of more severe seizures and cognitive, behavioral, and endocrine disorders.1 The imaging features help one to distinguish these conditions (figures 1 and 2). Isolated HH are hyperintense on flu...
متن کاملA case of pure gelastic seizures due to hypothalamic hamartoma with a benign course☆
Hypothalamic hamartoma is a potentially complex entity with diverse clinical manifestations. We report a case of gelastic seizures associated with a hypothalamic hamartoma, which followed a benign course. A 31-year-old woman with episodes of laughter was referred for diagnostic evaluation. Her initial MRI and EEG were reported as normal. However, her episodes of laughter were typical of gelasti...
متن کاملRespiratory Epithelial Adenomatoid Hamartoma: A Very Rare Entity Originating from the External Auditory Canal
Respiratory epithelial adenomatoid hamartoma (REAH) is an uncommon lesion of the upper aerodigestive tract. It is characterized by abnormal glandular formations with ciliated epithelium arising from the nasal cavity, paranasal sinuses and nasopharynx, but other locations have also been described. We did not find any other cases of this hamartomas in the external auditory canal in the literature...
متن کاملTorsional nystagmus in hypothalamic hamartoma.
Torsional nystagmus was noted in a patient with hypothalamic hamartoma. Magnetic resonance imaging revealed an exophytic hypothalamic mass extending into the pre-pontine cistern and abutting ventral mesencephalon. The quickphase of the torsional nystagmus was directed towards the left side, ipsilateral to the side of compression by the hamartoma. Ipsi-lesionally directed pure torsional nystagmu...
