Primary Stenting in Acute Myocardial Infarction

چکیده

With the demonstration that the timely administration of thrombolytic therapy after coronary arterial occlusion results in myocardial salvage and improved survival, the treatment of patients with evolving AMI has forever been changed. To overcome the inherent limitations of thrombolytic therapy (TIMI 3 flow rates of '55% at best, and rare but unavoidable life-threatening or incapacitating hemorrhagic complications), mechanical reperfusion by balloon angioplasty without antecedent thrombolysis (primary PTCA) has been adopted at many centers. Ten prospective, randomized trials comparing primary PTCA and lytic therapy in 2606 patients have now been performed and examined in a recent meta-analysis by Weaver et al, demonstrating that compared with thrombolytic therapy, primary PTCA results in reduced rates of mortality, reinfarction, and stroke. Other randomized trials have shown that primary PTCA, by reducing early and late recurrent ischemic events and facilitating earlier discharge, is as or more cost-effective than thrombolysis.

Randomized comparison of coronary stenting with balloon angioplasty in selected patients with acute myocardial infarction.

BACKGROUND Although the benefits of primary angioplasty in acute myocardial infarction have been demonstrated, several areas for improvement remain. Therefore, a prospective randomized trial comparing primary stenting with balloon angioplasty in patients with acute myocardial infarction was conducted. METHODS AND RESULTS Patients with acute myocardial infarction were randomly assigned to unde...

No-Reflow Phenomenon in Patients with ST-Elevation Acute Myocardial Infarction, Treated with Primary Percutaneous Coronary Intervention: A Study of Predictive Factors

Introduction: No-reflow phenomenon in coronary vessels, manifested in some patients with reperfused acute myocardial infarction (MI), is associated with poor clinical and functional outcomes. Therefore, evaluation of predisposing risk factors can be helpful in risk assessment and identification of patients at higher risk. Herein, we aimed to study the predictive factors for the development of...

Percutaneous coronary intervention in stable patients after acute myocardial infarction.

Reperfusion after acute ST-segment elevation myocardial infarction (AMI) remains the cornerstone of the initial therapy. Although reperfusion can be accomplished with either primary angioplasty or fibrinolytic therapy, clinical trial data reveal improved outcome with primary angioplasty. Furthermore, primary angioplasty with coronary stenting results in less repeat revascularization than balloo...

Long term outcome and cost-effectiveness of stenting versus balloon angioplasty for acute myocardial infarction.

OBJECTIVE To investigate the long term clinical outcome and cost-effectiveness of stenting compared with balloon angioplasty in patients with acute myocardial infarction. METHODS Patients with acute myocardial infarction were randomly allocated to primary stenting (112) or balloon angioplasty (115). The primary end point was the cumulative first event rate of death, non-fatal reinfarction, or...
