چکیده

Intracranial extension is the most dreaded complication of fungal sinusitis with high mortality rates. Fungal sinusitis is notorious for insidious and rapid intracranial spread. Therefore the most appropriate approach lies in timely diagnosis and early intervention. With various diagnostic modalities available; like CT scan, MRI, fungal stains, cultures and nasal endoscopy, the diagnosis has become much simpler, nevertheless, a high degree of clinical suspicion is a must. The treatment outcomes have now improved a lot, with the advancement in medical and surgical technology. The new antifungal agents and other drugs have immensely contributed to better results by minimizing the toxic side effects of the traditional drugs and showing better efficacy. The surgical approach, with the advent of endoscopes has led to successful outcomes. Despite this, in about 50% of the cases, since the host is immunocompromised, the prognosis is guarded and mortality rates of approximately 85 to 100% are noted. So, it is very important to treat intracranial extension of fungal sinusitis as a medical and surgical emergency.

Temporal lobe abscess in a patient with isolated fungal sphenoiditis.

Pathology References Isolated sphenoid sinusitis accounts for 2-3% of all sinus infections evaluated by radiologic imaging. Fungal sphenoiditis is present in approximately 5% of isolated sphenoiditis. The most common presenting symptom is frontal/retro-orbital headache; this occurs in roughly 70% of cases. Nasal obstruction is the next most common symptom presenting in 30% of cases. Noninvasive...

OBJECTIVE This study was conducted to present the clinical outcome of invasive fungal sinusitis of the sphenoid sinus and to analyze clinical factors influencing patient survival. METHODS A retrospective review of 12 cases of invasive fungal sphenoiditis was conducted. RESULTS Cases were divided into acute fulminant invasive fungal spheonoidits (n=4) and chronic invasive fungal sphenoiditis...

Allergic fungal sinusitis: innocence under suspicion.

The objective of this investigation was to explore the possibility of treating patients harboring invasive intracranial aspergillosis (InIA) at an early stage. Nineteen patients (age range 18-42 years) from a total of 114 cases of InIA seen from January 1999- December 2009 were included in this investigation. These individuals, all of whom had a past history of treated allergic fungal sinusitis...

Acute MRSA Sinusitis with Intracranial Extension and Marginal Vancomycin Susceptibility

Methicillin resistant Staphylococcus aureus (MRSA) is increasingly being described as a cause of acute sinusitis. We present a patient with acute MRSA sinusitis complicated by rapid intracranial extension, marginal vancomycin susceptibility (MIC = 2 mg/L), delayed drainage of intracranial abscess, and subsequent development of rifampin resistance. Given the relatively high risk of intracranial ...

Survival outcomes in acute invasive fungal sinusitis: A systematic review and quantitative synthesis of published evidence.

OBJECTIVES/HYPOTHESIS Acute invasive fungal sinusitis (AIFS) is an aggressive and often fatal infection. Despite improvements in medical and surgical therapy, survival remains limited and the factors that contribute to patient outcomes remain poorly understood. The current study systematically reviews and quantitatively synthesizes the published literature to characterize prognostic factors ass...
