Dietary modifications: food dependent autoimmunity in coeliac disease.

چکیده

The action of tissue transglutaminase (TGase) on specific protein-bound glutamine residues plays a critical role in numerous biological processes. Here we provide evidence for a new role of this enzyme in the common, HLA-DQ2 (and DQ8) associated enteropathy, celiac disease (CD). The intestinal inflammation in CD is precipitated by exposure to wheat gliadin in the diet and is associated with increased mucosal activity of TGase. This enzyme has also been identified as the main target for CD-associated antiendomysium autoantibodies, and is known to accept gliadin as one of its few substrates. We have examined the possibility that TGase could be involved in modulating the reactivity of gliadin specific T cells. This could establish a link between previous reports of the role of TGase in CD and the prevailing view of CD as a T-cell mediated disorder. We found a specific eVect of TGase on T-cell recognition of gliadin. This eVect was limited to gliadin-specific T cells isolated from intestinal CD lesions. We demonstrate that TGase mediates its eVect through an ordered and specific deamidation of gliadins. This deamidation creates an epitope that binds eYciently to DQ2 and is recognized by gutderived T cells. Generation of epitopes by enzymatic modification is a new mechanism that may be relevant for breaking of tolerance and initiation of autoimmune disease. Coeliac disease: a unique model for investigating broken tolerance in autoimmunity

Coeliac disease, a prevalent immune-mediated enteropathy driven by dietary gluten, provides an exceptional human model to dissect the genetic, environmental and immunologic factors operating in autoimmunity. Despite the causative antigen being an exogenous food protein, coeliac disease has many features in common with autoimmune disease including a strong HLA class II association and the presen...

Coeliac disease and autoimmunity.

Among fifty-seven patients with proven coeliac disease three also had thyrotoxicosis, two insulin dependant diabetes, three fibrosing alveolitis and one each had vitiligo, rheumatoid arthritis and cryptogenic cirrhosis. Autoantibodies were demonstrated in twelve of thirty-one patients tested, seven of these twelve had one of the above associated conditions. Antireticulin antibodies were found i...

Duration of gluten exposure in adult coeliac disease does not correlate with the risk for autoimmune disorders.

BACKGROUND AND AIMS Duration of gluten exposure seems to predispose adolescents with coeliac disease to autoimmune diseases. In a retrospective cohort study, we assessed the relationship between autoimmune disorders and actual gluten exposure in patients in whom coeliac disease was diagnosed in adult life (> or = 16 years). METHODS We screened for the presence of autoimmunity in 605 controls ...

Out of 55 patients with coeliac disease, 40 had food antibodies in their plasma at diagnosis. The antibodies disappeared in all but 6 of 21 patients after 2 months on a gluten-free diet. 5 out of these 6 patients had h...
