The Treatment of Large and Giant Paraclinoid Internal Carotid Artery Aneurysms

چکیده
Paraclinoid internal carotid artery aneurysms originate from the internal carotid artery between the distal dural ring and the posterior communicating artery. Surgical treatment of these aneurysms is technically challenging because of the surrounding neurovascular structures. There are several ways to treat these lesions: direct clipping with retrograde suction decompression, endovascular coiling, coiling with stent, and flow diverter stent. Here, I briefly review the surgical and endovascular options. The Treatment of Large and Giant Paraclinoid Internal Carotid Artery Aneurysms

Internal Carotid Artery Reconstruction Using Multiple Fenestrated Clips for Complete Occlusion of Large Paraclinoid Aneurysms
OBJECTIVE Although surgical techniques for clipping paraclinoid aneurysms have evolved significantly in recent times, direct microsurgical clipping of large and giant paraclinoid aneurysms remains a formidable surgical challenge. We review here our surgical experiences in direct surgical clipping of large and giant paraclinoid aneurysms, especially in dealing with anterior clinoidectomy, distal...

OBJECTIVES The incidence of giant aneurysms in the paraclinoid section of the internal carotid artery (ICA) is low. They manifest their occurrence by subarachnoid haemorrhage and neurological and hormonal symptoms, resulting from compression on either the nervous structures or on the pituitary. The treatment of these aneurysms is a challenge for the surgeon, due to a difficult access, high oper...
متن کاملSurgical clipping is still a good choice for the treatment of paraclinoid aneurysms.
UNLABELLED Paraclinoid aneurysms are lesions located adjacent to the clinoid and ophthalmic segments of the internal carotid artery. In recent years, flow diverter stents have been introduced as a better endovascular technique for treatment of these aneurysms. METHOD From 2009 to 2014, a total of 43 paraclinoid aneurysms in 43 patients were surgically clipped. We retrospectively reviewed the ...
متن کاملClinical Outcome of Paraclinoid Internal Carotid Artery Aneurysms After Microsurgical Neck Clipping in Comparison with Endovascular Embolization
OBJECTIVE Because of the complex anatomical association among vascular, dural, and bony structures, paraclinoid internal carotid artery (ICA) aneurysms remain a major challenge for vascular neurosurgeons. We studied the clinical outcomes of 61 paraclinoid ICA aneurysms after microsurgical clipping in comparison with endovascular coiling. MATERIALS AND METHODS Between January 2008 and December...
متن کاملA combined microsurgical skull-base and endovascular approach to giant and large paraclinoid aneurysms.
BACKGROUND The treatment of giant and large paraclinoid aneurysms remains challenging. To improve exposure, facilitate the dissection of aneurysms, assure vascular control, reduce brain retraction and temporary occlusion time, enable simultaneous treatment of associated lesions, and achieve more successful treatment of "difficult" (atherosclerotic and calcified) aneurysms, we combined the skull...
