چکیده

A 42-year-old woman was referred for radiofrequency catheter ablation of narrow QRS tachycardia that was terminated with intravenous adenosine. Baseline twelve-lead ECG was normal during sinus rhythm and transthoracic echocardiography demonstrated no evidence of structural heart disease. The electrophysiological study showed a normal AH interval of 114 ms and HV interval of 37 ms during sinus rhythm. Atrial burst pacing reproducibly induced two tachycardia e 1) regular narrow QRS tachycardia and 2) wide QRS tachycardia with left bundle branch block (LBBB) morphology. Fig. 1 shows spontaneous transition of narrow QRS tachycardia to wide QRS tachycardia. What are the likely mechanisms of the tachycardia?

Rapid palpitations: three of a kind?

The first part of the ECG shows a wide QRS tachycardia with a right bundle branch block (RBBB) morphology, an inferior axis and a heart rate of 240 bpm (tachycardia cycle length (TCL) of 250 ms). The middle part shows a narrow QRS tachycardia with an intermediate axis and the same heart rate of 240 bpm (TCL 250 ms). In the last part a wide QRS tachycardia is observed, although with a different,...

Narrow QRS Tachycardia with Alternate Wide QRS Beats: What is the Mechanism?

A 44-year old lady underwent electrophysiology study for recurrent palpitations and documented narrow QRS regular tachycardia. The baseline ECG showed subtle preexcitation that was easily manifest on atrial pacing. The retrograde atrial activation sequence during ventricular pacing was eccentric suggesting retrograde conduction over the accessory pathway. A regular narrow QRS tachycardia with c...

Successful catheter ablation of focal ventricular tachycardia originating from right bundle branch without making right bundle branch block, using pharmacologic induction by landiolol

Introduction Right ventricular outflow tract tachycardia is one of the most common ventricular tachycardia (VT) observed in young patients, accounting for 60%–80% of all idiopathic VTs. The QRS morphology of this tachycardia is left bundle branch block (LBBB) type, inferior axis, and wide QRS complex. In contrast, focal VT originating from the right bundle branch (RBB) is a rare idiopathic VT. ...

A Case of Left Bundle Branch Block-shaped wide QRS Complex Tachycardia with diagnostic Ambiguity on a Surface Electrocardiogram

In cases of tachycardia with a broad QRS complex, it is important to differentiate between supraventricular tachycardia (SVT) and ventricular tachycardia (VT). Electrocardiogram (ECG)-based differential diagnoses include VT vs. SVT with aberrant conduction, pre-existing bundle branch block (BBB), intraventricular conduction disturbances, and pre-excitation. Several criteria have been described ...

tachycardias. When unsure of the type of tachycardia, it is wise not to use verapamil. Intravenous amiodarone or procainamide can be used. When a broad-QRS tachycardia shows RBBB-type of morphology in lead V1, a monophasic or biphasic complex in V1 and R/S <1 in V6, suggests VT. A triphasic complex in leads V1 and V6 suggests SVT. On the other hand, when lead V1 shows LBBB-type of morphology; a...
