Allogeneic Blood Stem Cell Transplantation: Peripheralization and Yield of Donor-Derived Primitive Hematopoietic Progenitor Cells (CD34+ Thy-ldim) and Lymphoid Subsets, and Possible Predictors of Engraftment and Graft-Versus-Host Disease

چکیده
Apheresis-derived hematopoietic progenitor cells have recently been used for allogeneic transplantation. Forty-one normal donors were studied to assess the effects of recombinant human granulocyte colony-stimulating factor (rhGCSF) (12 pglkgld) on the peripheralization of hematopoietic progenitor cells and lymphoid subsets. The white blood cell, polymorphonuclear cell (PMNC), and lymphocyte concentrations at the peak of rhG-CSF effect in the donor's peripheral blood (PB) exceeded baseline by 6.4-, 8.0-, and 2.2-fold, respectively. Corresponding concentrations of PB CD34' cells and primitive subsets such as CD34' Thy-ldim, and CD34' Thy-l""' CD38cells increased by 16.3-fold, 24.2-fold, and 23.2-fold. respectively in eight normal donors. The percentage of CD34+ Thy-ldim and CD34+ Thy-ldim CD38cellsamong CD34+ cells increased as well, suggesting an additional peripheralization effect of rhG-CSF on primitive CD34'subsets. The preapheresis PB CD34' and CD34' Thy-ldim cell concentrations were predictive of their corresponding apheresis yield per liter of donor blood processed. PB lymphoid subsets were not significantly affected by rhG-CSF treatment. The mean apheresis-derived yield of CD34+, CD34+ Thy-ldi", and CD34' Thy-ldim CD38cells per kilogram of recipient body weight and per liter of donor blood processed was 48.9 x I O 4 (n = 41). 27.2 x IO4 (n = IO), and 1.9 x I O 4 (n = IO), respectively. As compared with 43 single bone marrow (BM) harvests, the CD34' cell yield of peripheral blood progenitor cell allografts of 41 normal donors exceeded that of BM allo-

Assessment of Cyclosporine Serum Concentrations on the Incidence of Acute Graft versus Host Disease Post Hematopoietic Stem Cell Transplantation
Allogeneic hematopoietic stem cell transplantation (HSCT) is a curative treatment option for hematological disorders. Cyclosporine (CsA) is one of the major immunosuppressive agents for the prophylaxis against graft versus host disease (GvHD). In this retrospective study, we evaluated the effects of CsA serum levels on the incidence of acute GvHD and transplant outcomes. 103 adult patients rece...

Background : Allogeneic hematopoietic stem cells transplantation (HSCT) is a valuable therapy for refractory acute leukemias, leukemias with a high risk for relapse, myelodysplastic syndromes, and chronic myeloid leukemia. HSCT outcome is dependent on several factors, including the stage of disease, degree of human leukocyte antigen (HLA) identity between donor and recipient, conditioning regim...
متن کاملAllogeneic T cells impair engraftment and hematopoiesis after stem cell transplantation.
Because of the perception that depleting hematopoietic grafts of T cells will result in poorer immune recovery and in increased risk of graft rejection, pure hematopoietic stem cells (HSC), which avoid the potentially lethal complication of graft-versus-host disease (GVHD), have not been used for allogeneic hematopoietic cell transplantation (HCT) in humans. Ideal grafts should contain HSC plus...
متن کاملMultiple allogeneic progenitors in combination function as a unit to support early transient hematopoiesis in transplantation
Cord blood (CB) is a valuable donor source in hematopoietic cell transplantation. However, the initial time to engraftment in CB transplantation (CBT) is often delayed because of low graft cell numbers. This limits the use of CB. To overcome this cell dose barrier, we modeled an insufficient dose CBT setting in lethally irradiated mice and then added hematopoietic stem/progenitor cells (HSCs/HP...
متن کاملPossible Role of Minor H Antigens in the Persistence of Donor Chimerism after Stem Cell Transplantation; Relevance for Sustained Leukemia Remission
Persistent complete donor chimerism is an important clinical indicator for remissions of hematological malignancies after HLA-matched allogeneic stem cell transplantation (SCT). However, the mechanisms mediating the persistence of complete donor chimerism are poorly understood. The frequent coincidence of complete donor chimerism with graft-versus-leukemia effects and graft-versus-host disease ...
