چکیده

asymptomatic after the unipolar lead was changed to a bipolar system. Monitoring artificial pacemaker function is an important application of ambulatory electrocardiography. Of 48 patients with suspected pacemaker malfunction reported by Iyengar et al., 24-hour monitoring detected failure in nine in whom routine examination, standard electrocardiograms, and pacemaker evaluation clinics failed to detect the malfunction.2 One type of malfunction to which unipolar demand ventricular pacemakers are vulnerable is inhibition by skeletal muscle potentials. This phenomenon, if suspected, can be provo:ked by isometric arm exercise in up to 85% of patients with unipolar lead systems.3-5 The recording of myopotential inhibition by ambulatory electrocardiography, however, has not been previously documented. We appreciated this cause of pacemaker failure in our patient, only after we more carefully inspected a Holter monitor “artifact,” originally thought to be insignificant.

Echocardiography in the diagnosis of idiopathic hypertrophic subaortic stenosis coexisting with pericardial effusion.

Of 89 patients with idiopathic hypertrophic subaortic stenosis who had M-mode echocardiagrams recorded, seven patients with coexisting moderate to large pericardial effusions were identified. The clinical profile, M-mode echocardiograms, and cardiac catheterization and angiocardiographic studies in two of the seven patients were analyzed. That the two entities were not associated was suggested ...

surgery: etiologic classification by radionuclide ventriculography

Early after open heart surgery, cardiac shock due to tamponade is easily misdiagnosed as ventricular dysfunction. The distinction is critical to successful therapy. We assessed the utility of 99mTc-red blood cell, gated equilibrium radionuclide ventriculography in 50 patients with early postoperative cardiac shock after historical, clinical, and invasive hemodynamic evaluation failed to identif...

Potential pitfalls in quantification of pericardial effusions by echocardiography.

In the echocardiographic assessment of patients with pericardial effusions, the apparent width of the echo-free space between the left ventricular posterior wall and the parietal pericardium is commonly used to estimate the amount of pericardial fluid present. In 4 patients with pericardial effusions, we showed a distinct disparity between the widths of the posterior pericardial space at differ...

Assessment of cardiac function in patients with the acquired immunodeficiency syndrome.

We have assessed right and left ventricular function by multigated radionuclide ventriculography in 12 consecutive patients with acquired immunodeficiency syndrome (AIDS) grouped according to the CDC classification system for HIV infection. Results were correlated with clinical, electrocardiographic and echocardiographic findings. Clinical examination and chest x-ray films showed no evidence of...

Amoebic pericardial effusion--diagnosis suggested by ultrasonography.

A 35 years old male presented with a two months history of progressively increasing dyspnoea. On examination he was in respiratory distress, afebrile, had prominent neck veins and an increased area of cardiac dullness. The heart sounds were muffled, and the abdomen was unremarkable except for an enlarged, tender liver. The blood count was normal, but the ESR was raised (60mm). X-ray chest showe...
