During the period of 18 months from December 1972 to June 1974, eight patients WitJl suba rachnoid haemorrhage (SA H) were seen by the au thor in the Briti sh Military Hospitals at MUnster and Hannover in \Vc~t Germany. Spontaneous bleed ing had occurred in five patients. foul' of whom had so-called • berry' aneurysms and one a cerebral angiomatous malformation . Haemorrhage in the other three patients was attr ibuted to trauma.

We describe an obstetric patient who presented for removal of a retained placenta. After insertion of the spinal anaesthetic, she developed a severe headache, and a subarachnoid haemorrhage was diagnosed. We discuss the differential diagnosis of the headache, the occurrence of intracranial haemorrhages after dural puncture and the future management of this patient.
