Intrathoracic goitre in the posterior mediastinum.

چکیده
Several examination methods are now available for diagnosis of intrathoracic goiter; diagnosis of suspicion is made by conventional plain radiography of the thorax, clinical findings being often normal or non-characteristic. If the iodine scintigram shows accumulation in the tumor, we can be practically sure of the diagnosis of an intrathoracic goiter. In case of a negative iodine scan we can obtain pointers to the nature of the tissue via CT or MR, especially if a connection between cervical thyroid tissue and intrathoracic mass can be demonstrated. Furthermore, these two methods (CT and MR) enable exact determination of the extension of the goiter and show the topographical relation to the neighbouring organs or their displacement.

Management of intrathoracic goitre.
an intrathoracic goitre is a thyroid gland with more than 50% of its mass located below the thoracic inlet. This term can be further classified as an “incomplete” or a “complete” intrathoracic goitre. When the goitre is “incomplete,” a major portion of the goitre on the neck is visible, with its lower pole being substernal, subclavicular or intrathoracic. Conversely, a “complete” or “true” intr...

Unilateral phrenic nerve paralysis is a rare complication of cervico-mediastinal goitre. It occurs when adhesions grow between the intrathoracic part of the thyroid and the nerve, specially where the goitre enters the mediastinum behind the first rib. The damage may be caused by strain of the nerve due to the descent of the goitre into the chest or may be caused by the surgical manoeuvres durin...
متن کاملComplete excision of a giant thyroid goiter in posterior mediastinum
Intrathoracic goiter is commonly located in the anterior mediastinum. Here we report a case of a 58-year-old Chinese male in whom we successfully removed the intrathoracic goiter and eased his dyspnea by a right posterolateral thoracotomy approach. Posterior mediastinal thyroid goiter with mediastinal compressive symptoms is an indication of surgery.
متن کاملRetrosternal goiter with posterior mediastinal extension surgery in a patient with superior vena cava obstruction through a classic neck incision; Case report
Intrathoracic goiters are usually located anteriorly, in the superior or anterior mediastinum, and are termed substernal or retrosternal goiters. Posterior mediastinal goiters are rare, about 10% of all intrathoracic goiters. Patients with retrosternal goiter usually have a visible or palpable cervical mass. In addition, tracheal deviation may be present with compression symptoms. Posterior med...
متن کاملBest approach for posterior mediastinal goiter removal: transcervical incision and lateral thoracotomy.
Surgical removal of intrathoracic goiter can be performed by a cervical approach in the majority of patients. Review of literature shows that experienced surgeons need to perform an extracervical approach in 2-3% of cases. In spite of surgical management of substernal goiter is well defined, there is little available information about surgical approach of intrathoracic goiters extending beyond ...
