Another Milestone for Transcatheter Valve Therapy

چکیده
The first mechanical heart valve prosthesis, designed by Dr Charles Hufnagel, was implanted to the descending aorta of a 30-year-old woman with severe aortic regurgitation in 1952.1 Eight years after the initial successful prosthetic valve implantation, Dr Harken sutured a prosthetic valve (Starr Edwards Valve) to the aortic annulus after removing the diseased native valve. During the ensuing years, different types of valves made from pyrolytic carbon were tried. Though they successfully remedied the aortic valve disease, mechanical prosthesis required lifelong anticoagulation, resulting in high rates of bleeding and thrombosis complications.

Transcatheter aortic valve replacement (TAVR) is a novel therapeutic intervention for the replacement of severely stenotic aortic valves in high-risk patients for standard surgical procedures. Since the initial PARTNER trial results, use of TAVR has been on the rise each year. New delivery methods and different valves have been developed and modified in order to promote the minimally invasive p...

Transcatheter aortic valve replacement has a place in the therapy for valvular aortic stenosis in a selected population of patients with increased risk for standard aortic valve replacement. The SAPIEN family of balloon-expandable transcatheter heart valves is the prototype that initiated this therapy and has undergone rapid development and evolution. The SAPIEN system has taught cardiologists ...

TAVI (transcatheter aortic valve implantation) is a less invasive treatment of the stenotic aortic valve while avoiding midline sternotomy and cardiopulmonary bypass. A crimped biological valve on a self-expanding or balloonexpandable stent is inserted antegradely or retrogradely under fluoroscopy, and deployed on the beating heart. Among the worldwide TAVI programs, many different concepts hav...
