Tachycardia Discrimination Algorithms in ICDs

چکیده
The detection of cardiac arrhythmia and the differential algorithm used for the recognition of ventricular tachycardia (VT) or ventricular fibrillation (VF) necessitating internal cardiac de‐ fibrillator (ICD) therapy is one of the most critical issues in ICD patients. Main goal is to devel‐ op algorithms with the highest possible sensitivity to avoid untreated arrhythmias and highest possible specificity to avoid inappropriate ICD therapies. Currently, several device discrimi‐ nation algorithms and their combinations are available. The following chapter tries to explain the functionality, means, possibilities and restrictions of these ICD discrimination algorithms.

Dynamic analysis of cardiac rhythms for discriminating atrial fibrillation from lethal ventricular arrhythmias.
BACKGROUND Implantable cardioverter-defibrillators (ICDs), the first line of therapy for preventing sudden cardiac death in high-risk patients, deliver appropriate shocks for termination of ventricular tachycardia (VT)/ventricular fibrillation. A common shortcoming of ICDs is imperfect rhythm discrimination, resulting in the delivery of inappropriate shocks for atrial fibrillation (AF). An unde...

Atrial Sensing and Blanking in Dual-Chamber Implantable Cardioverter-Defibrillators Accurate atrial sensing during tachycardia is essential for reliable determination of atrial rate, which is a critical component of interval-based dual-chamber supraventricular tachycardia (SVT)–VT discrimination algorithms. Thus, atrial sensing in dual-chamber implantable cardioverter-defibrillators (ICDs) must...

Introduction Implantable cardioverter-defibrillators (ICDs) have been shown to improve survival in patients with ischemic or nonischemic cardiomyopathy when used for primary and secondary prevention settings. The limitations of transvenous ICDs include postimplant adverse events, defibrillator lead failure, and inappropriate shocks for atrial tachyarrhythmias due to fast ventricular response ra...

The discrimination of Ventricular Tachycardia (VT) from Supra-Ventricular Tachycardia (SVT) remains a major challenge for appropriate therapy delivery in Implantable Cardioverter Defibrillators (ICDs). Unlike SVT, VT is a life-threatening arrhythmia that may lead to sudden death unless an appropriate shock is delivered. The discrimination in ICDs is performed from endocardial measurements of th...

OBJECTIVES The OPTION (Optimal Anti-Tachycardia Therapy in Implantable Cardioverter-Defibrillator Patients Without Pacing Indications) trial sought to compare long-term rates of inappropriate shocks, mortality, and morbidity between dual-chamber and single-chamber settings in implantable cardioverter-defibrillators (ICDs) patients. BACKGROUND The use of dual-chamber ICDs potentially allows be...
