چکیده

In 78 consecutive patients, we performed fine-needle aspiration of the hip before revision surgery. At the revision operation biopsies were taken from the capsule and joint tissues. The aspirate and the homogenised soft-tissue specimens were cultured using a radiomefric technique. Fifteen hips proved to be infected and 63 sterile on culture of the tissues taken during revision surgery; 94% of the aspiration results were correct. The sensitivity of the technique was 87% and the specificity 95%. Fine-needle aspiration combined with a radiometric culture technique is a simple and reliable method of diagnosing infection of hip prostheses.

Infection following hip arthroplasties can present a diagnostic challenge. No test is 100 % sensitive and 100% specific; this prospective study was undertaken to evaluate the utility of FDG-PET imaging for diagnosing infected joint replacements. 24 hip joint replacements were studied prospectively and we have complete diagnoses with clinical signs and symptoms, laboratory tests, radiography, jo...

Review of the role of dynamic 18F-NaF PET in diagnosing and distinguishing between septic and aseptic loosening in hip prosthesis

Joint replacements may fail due to infection, dislocation, peri-prosthetic fracture and loosening. Between 0.4 and 4% of joint replacements are known to be complicated by infection and aseptic loosening 2-18%. Differentiating between infection and aseptic loosening has an important bearing on the ongoing strategy for antimicrobial therapy and surgical intervention, but distinguishing one from t...

A study of the wound environment during total hip arthroplasty.

Two hundred and seventeen total hip replacements were performed during a trial designed to assess the influence of a surgical isolator upon postoperative wound infection. The trial was controlled by performing 108 of the replacements in a standard plenumventilated theatre with twenty air changes/hr, which also housed the isolator procedures. Comparative postoperative wound infection rates were ...

The incidence of deep prosthetic infections in a specialist orthopaedic hospital: a 15-year prospective survey.

The Control of Infection Committee at a specialist orthopaedic hospital prospectively collected data on all episodes of bacteriologically-proven deep infection arising after primary hip and knee replacements over a 15-year period from 1987 to 2001. There were 10 735 patients who underwent primary hip or knee replacement. In 34 of 5947 hip replacements (0.57%) and 41 of 4788 knee replacements (0...

A Single Femoral Component for All Total Hip Replacements Performed by a Trust? Does This Affect Early Clinical and Radiological Outcomes?

Background: Hospitals may be under pressure to implement cost saving strategies regarding prosthesis choice. Thismay involve the use of components which are not the first preference of individual surgeons, or those they have littleexperience with. We aim to examine the effect of standardizing the type of femoral stem used in a single trust, anddetermine whether this is safe practice, particular...
