Hemostatic Agents in Neurosurgery

Hemostasis comes from the coordinated activation of platelets and plasma clotting factors to form a platelet fibrin clot. Two processes, primary and secondary hemostasis activation of the clotting cascade id done by collagen for the intrinsic pathway, and the extrinsic pathway is activated by the release of tissue factors from the damaged zone. The two converge ento the common pathway which begins with the conversion of Factor X to Xa, the conversion of prothrombin to thrombin, which is integral in clot stabilization via fibrin. This common pathway is facilitated by Factor V (Hawiger 1987).

In endoscopic neurosurgery problems with haemostasis due to poor access exist. We have developed a system which allows the delivery of a variety of haemostatic agents in a more efficacious manner. In a case of subarachnoid hemorrhage, the arteries of the circle of Willis were left undisturbed and undissected at postmortem examination. A block of frontal lobe tissue with the attached vessels was serially sectioned disclosing in its entirety a saccular aneurysm that had ruptured at the base rather than the dome.
