Fontan operation.

چکیده
D uring the past 4 decades, the Fontan operation has been carried out as a surgical treatment in patients with tricuspid atresia. Over the years, it has become established as a revolutionary therapy in patients with univentricular heart physiology. Many experimental and clinical studies have been performed with a view to understanding the Fontan circulation; in consequence, the operation has undergone several modifications and the prognosis of these patients has improved. However, Fontan circulation attrition still remains a problem; elevations in systemic venous pressure and pulmonary artery resistance, decreased preload and increased afterload, are factors that concern researchers in their efforts to improve the circulation. In this review, we focus on the problems of the operation and the questions arising from the study of Fontan circulation.

Abnormal patterns of intraventricular flow and diastolic filling after the Fontan operation: evidence for incoordinate ventricular wall motion.
OBJECTIVE To assess whether regional abnormalities of ventricular function are present in patients after the Fontan operation and to explore the implications of any such abnormalities for ventricular filling. DESIGN AND PATIENTS Prospective study in which 25 patients after the Fontan operation were compared with 25 healthy controls and 12 patients with a univentricular atrioventricular connec...

OBJECTIVE To study regional wall motion of the systemic ventricle in patients after the Fontan operation. DESIGN AND PATIENTS Systemic ventricular angiograms of 15 patients after the Fontan operation and of 11 unoperated patients with a univentricular atrioventricular connection were digitised frame by frame. Strict criteria for abnormal wall motion were used so that minor abnormalities were ...

Background: The Fontan operation is the definitive operation for palliation of complex congenital heart disease with single –ventricle physiology. The use of the extra cardiac conduit has recently been gaining popularity. The purpose of this study was to compare the outcomes of extra cardiac conduit Fontan procedure (off-pump technique) and that of traditional technique (lateral tunnel techniqu...
متن کاملResults of the modified Fontan procedure are not related to age at operation.
OBJECTIVE The modified Fontan procedure represents the final stage for the palliation of hearts with single-ventricle physiology. Different opinions exist regarding the optimal timing of the operation, with most centres advocating early intervention. By contrast, over the past decade, we have progressively increased the age at Fontan operation with the aim to potentially delay the onset of late...
متن کاملClinical outcome of patients 20 years after Fontan operation--effect of fenestration on late morbidity.
OBJECTIVE The Fontan operation has been proposed as definitive palliation for an increasing variety of hearts with complex univentricular anatomy, but late morbidity after Fontan operation is still a matter of concern. This retrospective study evaluates the late outcome in patients with Fontan circulation. METHODS We included 121 consecutive patients that underwent Fontan operation between 19...
