چکیده

40S The last decade has witnessed major improvements in local anesthetic injection and application techniques, which have permitted surgeons to do more and more under pure local anesthesia with minimal pain during the injection of the local agents. This has led to increase in patient safety and convenience while maintaining total patient perioperative comfort. Many procedures in hand surgery,1 cosmetic surgery,2 facial skin cancer reconstruction,3 and other procedures such as adult cleft lip repair4 can now be performed with the wide awake approach without sedation. For the patient, many of these procedures have been reduced to the simplicity of an office dental procedure. In addition, postoperative pain control with new developments in local anesthesia have also improved the patient experience. This article will review 2 exciting new areas of minimal pain local anesthetic injection techniques, new medications, and the best evidence to support the changing landscape of local anesthesia in plastic surgery.

Minimally Painful Local Anesthetic Injection for Cleft Lip/Nasal Repair in Grown Patients

INTRODUCTION There has been a recent interest in injecting large body and face areas with local anesthetic in a minimally painful manner. The method includes adherence to minimal pain injection details as well feedback from the patient who counts the number of times he feels pain during the injection process. This article describes the successes and limitations of this technique as applied to p...

Simple technique for lip and nasolabial fold anesthesia for injectable fillers.

BACKGROUND Minimally invasive cosmetic facial surgery procedures including injectable lip fillers are more popular than ever. Many of the newer fillers are more painful to inject than previous products and require a greater level of pain control. OBJECTIVE The objective of this article is to describe a simple method of local anesthesia infiltration for pain control when injecting filler subst...

Painful peripheral states and sympathetic blocks.

In various chronic painful states, the sympathetic nerve supply was blocked either by injecting the sympathetic chain and ganglia with local anaesthesia or by the injection of guanethidine during occlusion of the circulation. There was a striking relation between the presence of hyperpathia and the relief of pain by the blocks. The sympathetic block was unlikely to relieve the pain unless hyper...

Rapid local anesthesia in humans using minimally invasive microneedles.

OBJECTIVE This study tested the hypothesis that minimally invasive microneedles cause less pain during injection of lidocaine, but induce local anesthesia in humans with the same rapid onset and efficacy as intradermal lidocaine injection using hypodermic needles. METHODS This study was a randomized, single-blinded, within participants, controlled design. Hollow, 500-μm long microneedles were...

Minimizing injection pain in local anesthesia.

The choice of needle diameter can influence the pain felt by the patient. A well-designed randomized controlled trial (RCT) showed an inverse relation between needle diameter and pain. A smaller needle also forces the injector to slow down, minimizing pain from volume expansion. A longer needle (e.g., 1 1/4” v. 5/8”) is useful for injecting larger areas. In addition, warming the solution result...
